(Blood Disorders, Dysuria, and Hemorrhoids) Nineteen Sections in Total
*Chao Shi Bing Yuan* (Source of Diseases by Chao Yuanfang) on the Condition of Pediatric Hematemesis: When children exhibit hematemesis, it is due to excessive internal heat combined with blood deficiency. Heat pathogenic factors invade the blood; when blood is affected by heat, its nature becomes fluid and erratic, leading to reckless movement. If Qi rebels upward, blood follows Qi ascending, thus causing hematemesis.
Qian Yi discussed the differences between tonifying and purging treatments: The son of Duan Zhaolang, aged four, suffered from coughing, fever, and expectoration for several days, followed by hemoptysis. Physicians treated him with Jiegeng Tang (Platycodon Decoction) and Fangji Wan (Stephania Pill) without success. His phlegm-saliva attacked upwards, resulting in continuous vomiting and wheezing. He was referred to Qian Yi, who administered a large dose of Pianyin Wan (Biased Silver Pill) (see Phlegm-Saliva Section), followed by Bufei San (Lung-Tonifying Powder) and Bupi San (Spleen-Tonifying Powder) (see Stomach Qi Disharmony Section). Now, since the Duan family's child has hemoptysis and lung deficiency, why use purging methods? The response was: Although the lungs are bleeding, this is caused by heat. Prolonged illness leads to deficiency and atrophy. Currently, phlegm-saliva is surging upwards causing vomiting; one must purge the phlegm-saliva first. If the phlegm-saliva is not expelled, the condition will worsen. Vomiting phlegm can deplete healthy Qi and induce fright. Excess phlegm attacking upwards can also cause convulsions. Therefore, according to the method, one should first purge the phlegm, then tonify the spleen and lungs. Only when the phlegm stops will the vomiting cease. Tonifying the lungs first would be contrary to the principle; purging the phlegm first is in accordance with the principle. Purging first and tonifying later is the optimal approach.
*Yingtong Baojian* (Precious Mirror for Infants and Children) Song on Pediatric Hematemesis:
Blood belongs to Ying (Nutritive Qi), while Qi belongs to Wei (Defensive Qi). These two energies follow each other without stagnation. Suddenly, if Blood moves and Qi rushes against it, they recklessly deviate from their proper positions. Blood follows Qi upwards to the Heart, and the sight of vomiting blood is as gruesome as a slaughterhouse, hard to bear. One must administer medicine and provide careful nursing, resolving heat and following the correct sequence.
