Day, nine days, fourteen days: although the summer-heat pathogen is rampant and extremely severe, over time its force will inevitably weaken slightly. Sweating and fever are precisely the means to vent the summer-heat. The wind pathogen, which had not yet manifested, reveals itself at this point. With cold coming and heat going, and heat going and cold coming, treatment begins with a dual approach addressing both factors. The above cases involve an excess of summer-heat, differing from Dan Malaria (a type of malaria characterized solely by fever) which presents only with heat symptoms. Dan Malaria exclusively shows the specific signs and symptoms of Dan Malaria, whereas here it is merely that summer-heat exceeds wind and cold. Furthermore, there are cases where wind is dominant alone, showing only wind symptoms with absolutely no signs of summer-heat; treatment should follow wind alone, without needing to address summer-heat. Such cases are termed by people as Cold Damage (Shanghan) or Wind Strike (Zhongfeng), or after a long time when both wind and summer-heat appear, rumors claim "wind-cold transforms into malaria," which is also erroneous. This is because cold and wind pathogens, if contracted in winter, cause immediate illness, each manifesting transformations of their root and branch; those that do not cause immediate illness transform into Warmth diseases in spring and Summer-heat diseases in summer. Summer-heat disease involves extreme heat heavier than warmth disease; upon initial manifestation, there is no aversion to cold, only aversion to heat, accompanied by irritability, thirst, and absence of sweating. If wind transforms into summer-heat disease by summer, it adds spontaneous sweating, snoring respiration, difficulty speaking, floating pulse, and heavy body. Thus, these warmth and summer-heat diseases occur only in spring and summer, changing with the times, and manifesting symptoms accordingly, hence named according to the season. By summer, the body's mechanism for releasing pathogens is fully active, leaving no room for hidden storage; thus, pathogens must emerge completely following the summer release mechanism. It is unheard of for them to remain hidden until autumn, only to show winter-onset cold transformation without any branch-related yang manifestations. The Classics state: "In summer, one is injured by summer-heat; in autumn, malaria forms." This is like cold wind transforming into warmth in spring and summer-heat in summer—the mechanism is the same. From this, we see where summer's cold wind comes from; how can one say cold wind transforms into malaria? This ignorance arises because although summer injuries involve summer-heat, they are not as fierce as autumn winds; thus, only cold wind is seen, with no summer-heat signs. Similarly, when summer-heat burns fiercely, fluctuating in intensity or worsening every seven, nine, or fourteen days, wind虽有摧拉 (though destructive), its force weakens by then; generating heat and sweat serves to vent the wind. The hidden summer-heat and wind begin without dominance, achieving balance and mutual manifestation, allowing for dual treatment. These cases involve exclusive wind command, differing from Pin Malaria (a type of malaria characterized solely by chills) which presents only with cold symptoms. Pin Malaria exclusively shows Pin Malaria signs; here, wind exceeds summer-heat. If summer-heat invades the yin level, treatment methods are similar; however, summer-heat resides deeply, differing from superficial conditions. One must first assess the victory/defeat of the two pathogens. If biased toward the Ying (Nutritive) level, adjust the main formula based on symptoms; subsequently treat summer-heat using herbs like Shi Gao (Gypsum Fibrosum) and Jing Mi (Oryzae Semen) to stabilize the shifting yin-yang. "Chang" implies constancy/unchanging; "Shan" implies Gen trigram stillness/non-migration. Cases occurring every two days or several days can be inferred analogously. If accompanied by Foot-Taiyang meridian signs, the pathogen resides in the Foot-Taiyang; use Guizhi Chaihu Ge Ban Tang (Cinnamon Twig and Bupleurum Half-Half Decoction). Taiyang governs opening; alternating chills and fever seem pivot-like. Thus, treat via pivot-to-opening; if severe, Da Qing Long Tang (Major Green Dragon Decoction) is indicated. Taiyang follows both root and branch, so it can follow the root qi of wind-summer-heat or the branch manifestation of cold-transformation yang images. If accompanied by Foot-Yangming meridian signs, the pathogen resides in Foot-Yangming; use Guizhi Yi Baihu Er Tang (One Part Cinnamon Twig, Two Parts White Tiger Decoction). Yangming governs closing; treating the opening allows evil to depart, restoring closure. Doubling Baihu Tang is because Yangming does not follow root/branch but treats the middle damp-transformation. If severe, Biejia Jian Wan (Turtle Shell Pill for Reducing Masses) is used due to Yangming's abundance of blood and qi. If Stomach Fullness (Wei Jia Shi) with difficult defecation occurs, Tiaowei Chengqi Tang (Regulate the Stomach and Purge the Bowels Decoction) is indicated. If accompanied by Foot-Shaoyang meridian signs, the pathogen resides in Foot-Shaoyang; use Xiao Chaihu Tang (Minor Bupleurum Decoction) because Shaoyang follows the root and governs the pivot. If accompanied by Foot-Taiyin meridian signs, the pathogen resides in Foot-Taiyin; use Xiao Jianzhong Tang (Minor Construct the Middle Decoction) because Taiyin governs opening and follows the root. If spontaneous diarrhea with pus/blood and abdominal pain occurs, Guizhi Bei Shaoyao Jia Dahuang Tang (Cinnamon Twig with Doubled Peony and Added Rhubarb Decoction) is used because Spleen Fullness requires expelling putrid filth. If accompanied by Foot-Shaoyin meridian signs, the pathogen resides in Foot-Shaoyin; use Chaihu Jia Xixin Tang (Bupleurum with Asarum Decoction) because Shaoyin is the pivot and follows root/branch. If accompanied by Foot-Jueyin meridian signs, the pathogen resides in Foot-Jueyin; use Siwu Jia Kulian Fuzi Huangqin Tang (Four Substances with Melia, Aconite, and Scutellaria Decoction) because Jueyin governs closing, does not follow root/branch, but treats the middle fire-transformation. If severe cold limbs occur, purgation is needed because deep cold implies deep heat. If thirst, Qi rushing to the heart, burning pain in the heart, hunger but inability to eat, vomiting roundworms after eating occur, Wu Mei Wan (Mume Pill) is indicated. If accompanied by Lung signs, the pathogen resides in the Lung Shu-Mu point; initially use Guizhi Huangqin Baiwei Kuandonghua San (Cinnamon Twig, Scutellaria, Anemarrhena, and Coltsfoot Powder); subsequently use Shumi, Gancao, Changshan (Polygoni Multiflori Caulis). If accompanied by Heart signs, the pathogen resides in the Heart Shu-Mu point; initially use Guizhi Huangqin Tang (Cinnamon Twig and Scutellaria Decoction); subsequently use Gancao, Shuqi, Changshan, Biejia, Shi Gao, Jiangcan (Fermented Soybean), Zhizi, Wumei, Dan Zhuye (Lophatherum Leaf). If accompanied by Spleen signs, the pathogen resides in the Spleen Shu-Mu point; initially use Xiao Jianzhong Tang; subsequently use Jingmi, Changshan, Gancao, Zhimu, Biejia. If accompanied by Liver signs, the pathogen resides in the Liver Shu-Mu point; initially use Tongmai Sini Tang (Pulse-Unblocking Reversal-Cold Decoction); subsequently use Wumei, Shuqi, Biejia, Nuwei, Zhimu, Kushen, Changshan, Shi Gao, Gancao, Xixin, Baiwei, Jiangcan. If accompanied by Kidney signs, the pathogen resides in the Kidney Shu-Mu point; initially use Guizhi Jia Danggui Shaoyao Tang (Cinnamon Twig with Added Angelica and Peony Decoction); subsequently use Zhuye, Changshan, Wumei, Jiangcan, Congbai. If accompanied by Stomach signs, the pathogen resides in the Stomach Shu-Mu point; initially use Guizhi Er Baihu Jia Shaoyao Huangqin Mugui Tang (Two Parts Cinnamon Twig, One Part White Tiger plus Peony, Scutellaria, and Oak Twig Decoction); subsequently use Lilü, Changshan, Zaojiao, Niuxi, Badou. For Wen Malaria (warm malaria) with heat preceding chills: First, Guizhi Er Mahuang Yi Tang (Two Parts Cinnamon Twig, One Part Ephedra Decoction); Second, Kuandong Baiwei Ru Yuan (Coltsfoot and Anemarrhena Pill). For Han Malaria (cold malaria) with chills preceding heat: Ma Huang Er Guizhi Yi Xiao Qing Long Yi Tang (Two Parts Ephedra, One Part Cinnamon Twig, One Part Minor Green Dragon Decoction). For Dan Malaria with heat only and no chills: First, Nuwei Shi Gao Tang (Anemarrhena and Gypsum Decoction) or Gegen Zhuling Tang (Kudzu and Polyporus Decoction); Second, Jiangcan Zhizi Gualou Tang (Fermented Soybean, Gardenia, and Trichosanthes Decoction). For Pin Malaria with chills only and no heat: Shuqi Tang (Vinegar-processed Polygonum Decoction) or Muli Tang (Oyster Shell Decoction). If autumn onset with severe chills: Initially follow wind to support Yang, then use the main dual-treatment formula. If winter onset with mild chills: Initially use Huoxiang Zhengqi San (Agastache Powder for Rectifying the Qi), then the main formula may suffice. Emphasis should be on following wind rather than summer-heat, adhering to the principle of assessing annual climate and preserving Heavenly Harmony. If spring onset with aversion to wind: Initially use Zaipo Tang (Bupleurum and Magnolia Decoction), then the main formula may suffice. If summer onset with profuse sweating: The main formula is primary; even during summer-heat epidemics, emphasize following wind until wind and summer-heat are balanced and root/branch mutually manifest, then apply dual treatment. If the disease reverses its root and transforms into branch-yang, emphasize summer-heat in the main formula; if severe, use only branch formulas focusing on summer-heat, such as Baihu Tang (White Tiger Decoction) or Zhuye Shigao Tang (Lophatherum and Gypsum Decoction). Assess internal/external signs: if cold has ceased, and during fever there is great irritability/thirst, profuse sweating, facial/dental grime, unquenchable thirst for cold drinks, then administer. If summer-heat signs exist, facial/dental grime present, but during fever, ice water cannot cool, and shivering aversion to wind persists, revert to main formula emphasizing wind. Another case: shivering aversion to wind or slight aversion to cold, unlike deficiency-cold exterior weakness, but heat intensifies with cold wind exposure; this is Yin deficiency and Yang hyperactivity, emphasizing summer-heat. Another case: extreme cold turning to heat, extreme heat turning to sweat, extreme sweat failing to resolve heat; exterior remains, emphasizing wind. If pulse is rapid/deep, delirious speech, silence/unwillingness to speak, unrelieved by sweat, the disease is Yin-Yang Intersection (Yin Yang Jiao), which is fatal. Another case: extreme heat/sweat fails to resolve heat, or thirst/no thirst, limb pain/irritability, tendon spasm, continuous steaming sweat; this is cold attacking Ying Qi transforming into blazing inflammation; if treated by wind, oral/gum ulcers and sudden death ensue. Method: soothe Ying Qi to moisten sinews. If heavy body/stiffness/muscle swelling occurs, this is Wind-Damp binding; method: soothe Wei Qi to fill muscle interstices. Another case: exterior sweating/aversion to heat, interior Stomach Fullness/difficult defecation; this is Yangming internal knotting. Another case: sweating/irritability/panting/thirst for cold water, white dry tongue coating, drinking immediately drying up; this is Heat in the Chest. If white slippery coating, this is Cold in the Upper Chest. Another case: sweating/irritability/panting/thirst for cold water, chest fullness, palpitations, vomiting/hiccup/cough/choking; this is Water Retention. If water medicine vomited immediately, this is Water Counterflow. Another case: post-profuse sweating, dry stomach, irritability/insomnia, desire for water; give small sips to harmonize Stomach Qi. If floating pulse, slight fever, thirst; this is Dryness of Chest Water. If empty irritability/insomnia/tossing-turning/regret; this is Guest Heat in the Chest. Another case: sweating/irritability/panting/thirst for boiling water, white slippery tongue coating; this is Cold in the Stomach. If gray-white coating, frequent drinking/drying; this is Heat in the Stomach. Another case: white slippery, greasy turbid tongue coating; this is undigested food retention. In Upper Jiao, induce vomiting; in Stomach, purge. If current diarrhea/anorexia/aversion to food odor/hard belching; also undigested food. If treated by wind, continuous sweating/cold limbs/dry stomach; even with Chengqi pills, hard to reach intestines. Another case: dry teeth/tongue, thirst for cold/boiling water, but internal Yin-Cold; do not assume heat. Food/drink retention causing lack of saliva leading to dryness; nasal blockage/lip peeling causing mouth breathing leading to dryness. Another case: blue-black tongue coating; do not assume heat. Sweet/sour foods may cause blackening, not natural color. Another case: no tongue coating, smooth/thin/red-white tender skin, looking moist but wiping reveals extreme dryness; caused by reckless sweating/vomiting/purging depleting blood; fatal. Above are variations of cold/heat/empty/full; appended for reference. For chronic unresolved disease forming masses (Zheng Jia): urgently treat with Biejia Jian Wan. For Consumption (Xu Lao): Shuqi Wan, Niuxi Tang, or Dingxiang Wine. If summer-wind excess paralyzes meridians; or cold/heat changes disrupt ascending/descending; or empty/full alternate disrupting upper/lower; or exterior Yang dissipates while interior Yin sinks; or Ying/Wei depletion damages blood/qi; or old disease remains while new evil attacks; or gluttony causes fluid/food retention; or lust exhausts essence thinning form; or fatigue exhausts spirit; or anger severs qi/form; or worry injures Spleen; or cold drink injures Lung; or forced effort; or sweat exposed to wind; or old disease reappears while new seems hidden; or new cures while old flares; or apparent right/wrong; all cause chronic disease. Assess emotional desires for tonification/purging and severity to determine nature. Lung Metal Dryness stagnation and Ying Blood obstruction without Lung/Blood branch signs indicate Heaven-formed Qi stagnating, becoming Earth Fire, punishing Metal organs, leading to subsequent diseases. Fire punishing Metal: shortness of breath/cough, bleeding, rhinitis/sore throat, deafness/red eyes, shoulder/back heat, urticaria/prickly heat, skin pain/oozing; severe: scorched lung/fullness, stabbing chest pain, dry cough/phlegm (Lung萎萎); pus/bloody foul sputum (Lung Abscess). Subsequent: Blood obstruction becomes Consumption, deficient vessels fail to nourish organs, numbness, rough skin, dark eyes, dizziness, hair loss, limb pain/fever, panting on movement, cold limbs, nosebleeds/hemoptysis, palpitations, dry throat/tongue, urgency, lower abdomen hardness, urinary difficulty, back pain, cold genitals, clear semen, nocturnal emission, soul loss, loose stools, indigestion, borborygmus, abdominal distension, night sweats, insomnia, scrofula/goiter. Wind combining with Wei Qi causes chills/jaw quivering and quick changes, reflecting Wind nature. Spleen Earth Dampness stagnation and Wei Qi obstruction without Spleen/Wei branch signs indicate Heaven-formed Qi stagnating, becoming Earth Wood, overcoming Earth organs, leading to subsequent diseases. Wood punishing Earth: jaundice/dysentery, emaciation, poor nutrition, heavy body/irritability, borborygmus/distension, stiff tongue/saliva; severe: Sanjiao failure, four pillars severed, emaciated limbs/swollen feet. Subsequent: Qi obstruction becomes Consumption, nine orifices closed, muscles blocked, Wei Qi dissipates, shortness of breath, hair loss, blood pooling above; Yin failing Yang: rapid pulse/manic behavior; Yang failing Yin: organ conflict, orifice blockage, blindness/deafness, collapse. Water Qi residing in skin with Wei Qi causes Dampness. If Dampness not expelled: whole-body pain, edema/distension, head sweat/heavy head, yellow body/back stiffness, desire for warmth; severe: muscle atrophy/spasms/paralysis, joint yellow sweat, abdominal distension/anorexia/diarrhea. Early intervention turns Dampness to Earth, punishing Water organs: abdominal pain/cold limbs/depression/heavy body/chest discomfort, impotence/back/lower abdominal pain, epigastric pain/mobility issues, food intolerance/cold calves. Summer cold water stored in interstices causes Cold Transformation. If Cold not broken: Water loses function, Heart Qi suppressed, palpitations/sweating/aversion to wind; severe: asthma/wheezing, pleural effusion, eye puffiness/leg/abdominal swelling. Early intervention turns Cold to Water, transmitting to Fire organs: fever/restlessness/palpitations, bone/muscle separation, leg weakness/diarrhea/bleeding, Yin reversal/runaway gas, internal cold, chest pain/heat/confusion/thirst. If Summer-heat resides in Foot-Taiyang boundary: heavy head/back pain, cold starting from back, chills then heat, lingering sweat. Untreated: enters meridians/fu organs. Head/neck/spine pain, eye pain, tearing, rhinorrhea/epistaxis, urinary retention, upward staring/incontinence. If in Foot-Yangming boundary: chills, late heat, sweating preferring light/fire. Untreated: enters meridians/fu organs. Dark face/yawning, fear of people/fire/wood sound, restlessness, hiding, climbing/singing/naked running, abdominal rumbling/distension, mania/sweating/rhinorrhea, lip sores/neck swelling/hoarseness, ascites/knee pain, leg/foot pain, middle finger dysfunction, stomach heat/cold, menstrual issues, wind consumption/lung inflation. If in Foot-Shaoyang boundary: body relaxation, mild chills/heat, fear of people, palpitations, prolonged heat/sweat. Untreated: enters meridians/fu organs. Bitter taste/sighing, chest/flank pain, turning difficulty, dusty face/lusterless skin, foot heat, toe dysfunction, head/jaw/eye pain, clavicle/chest/flank/knee/ankle pain, alternating chills/heat, scrofula/goiter, confusion/yellow discharge. If in Foot-Taiyin boundary: depression/sighing, anorexia, chills/heat, sweating, vomiting improving after episode. Untreated: enters meridians/zang organs. Stiff tongue, epigastric pain, abdominal distension, belching/vomiting, relief after bowel movement/gas, heavy body, big toe dysfunction, tongue pain, immobility, anorexia, irritability, acute epigastric pain, loose stools/dysentery, urinary obstruction/jaundice, inability to lie down/sleepiness, thigh/knee swelling, cold limbs/weakness. If in Foot-Shaoyin boundary: severe vomiting, chills/heat (more heat), desire to hide. Untreated: enters meridians/zang organs. Throat/jaw swelling, neck turning difficulty, shoulder/heel pain, deafness/red eyes, cheek/neck/shoulder/elbow/arm pain, lower abdominal pain, heavy diarrhea, cold calves/bone weakness.
