The child receives various pathogenic heat in the womb; heat stagnates in the Sanjiao (Triple Burner: upper, middle, and lower abdominal cavities) causing Chongshe (Double Tongue), or forms E Kou (Goose Mouth Thrush), which is worrying. Medication should be accompanied by acupuncture to lacerate and drain.
Constitutional Yuan Qi (Original Qi) is deficient and marrow is not filled; the Six Excesses (Wind, Cold, Summerheat, Dampness, Dryness, Fire) easily attack. The head, hands, feet, and body are emaciated and weak; this syndrome is named Wu Ruan (Five Softnesses).
Suddenly the Tian Zhu (Heavenly Pillar: neck/spine) collapses, what then? This disease is due to Liver and Kidney deficiency. External Wind pathogens easily invade, thus transmitting to the neck causing softness and inability to move freely.
The skull initially appears as if sutured open (Jie Chu); Kidney weakness and Yuan Qi deficiency are greatly concerning. Tonifying the Kidney and regulating Yuan Qi is the good medicine; if it does not respond, recovery will surely be difficult.
Pediatric spontaneous sweating has many manifestations; do not view it as a single case. One must distinguish Yin-Yang and Deficiency-Excess syndromes; diligent regulation leads to natural peace.
Body yellowing is caused by Summerheat and Dampness steaming the Spleen; treating internal and external causes separately is best. There are also congenital cases visible at birth; the mother should take Dihuang Tang (Rehmannia Decoction) frequently.
Why do the Nine Pathways experience reckless blood movement? It is likely due to depression generating Heat pathogens. Treating according to the meridians and clarifying Deficiency-Excess allows Qi to flow smoothly and Blood to harmonize naturally.
Once the human body suffers injury, severe cases involve much bleeding. Non-internal, non-external causes have their own logic; each syndrome is treated within its specialty.
To keep children safe through the four seasons, one must maintain a state of slight hunger and cold. If everyone follows this method, naturally various diseases will not interfere.
The above poetic verses, though plain, are valuable for being easy to remember. What is grasped in the heart responds in the hand; what cannot be achieved? Referencing the subsequent discussions reveals greater detail. This is specifically to encourage beginners, not to claim originality over others.
(Using "Essential Recognition of Forms and Syndromes in Pediatric Specialty" as the rhyme scheme, following the eight-rhyme fu format used in imperial examinations.)
Carefully distinguishing various syndromes makes pediatric conditions hard to clarify. Only by exploring medical principles can one achieve understanding, manifested in the specialty. Wind-cold with clear nasal discharge indicates initial transmission to the Lung; food stagnation with yellow lines indicates involvement of the Spleen. Within seven days, umbilical pain is common; after one hundred days, bone-steaming fever is rare. For fright, malnutrition, accumulation, and heat, half requires purging the interior. Thus, for Summerheat, Dampness, Wind, and Cold, releasing the exterior is first priority. Generally speaking of thirteen categories, each requires specialization. Regardless of age, all are called "small." Observing sick infants requires pulse analysis. Although pain and itch are hard to ask about, severity is clearly observed across meridians. Sudden pathogenic invasion shows slightly purple cheeks; long-term disturbance by external evil shows a clear Shan Gen (root of nose). Do not ask about the child's age, but rely on effective herbs. If Kidney pathology attacks, black color often floats on Di Ge (Chengjiang point area). If Heart is overcome by evil, red lines appear uniquely on Tian Ting (forehead center). Here, pulse, disease, signs, and treatment require detailed examination. Infants change unpredictably. All suffering relies on medical insight. Without specializing in one field, how can efficacy be achieved? If a doctor does not come from three generations of practice, effort is futile. It is difficult to match the rash discussion of beginners; value lies in the standardized approach of the specialty. When Heart and Liver are tranquil, they naturally avoid seasonal disasters. When Spleen and Stomach are harmonious, night eating is avoided. Let us speak of medicine connecting to the Dao of immortality, broadly extending life. Family secret formulas and treatments for youth participate in the transformation and nourishment of Heaven and Earth, without pause, relieving parents' worries. With fright, recovery occurs. Even using sage-like drugs depends on professional specialization. However, seeing Yu Kou (fish-mouth anus prolapse) indicates extremely weak Qi; hearing abnormal sounds inevitably leads to death. Since danger is revealed truthfully, why complain and spread falsehoods? Exterior use involves unblocking; detailing Master Dai's words. Interior organs clarify tonification and sedation, referencing Qian's chapters. What is specialization? Precise meaning without duality. Saving children, broad learning lies in understanding key mechanisms. Knowing the small pulse department, one often likes multiple effects of one drug; the large prescription department inherently has ten forms and three therapies. Feeding with milk and food requires moderation, avoiding drumming medicines to prevent fright. Harmonizing from childhood ensures no illness; why worry about becoming unworthy when grown? A spring-like thought universally promotes sincerity in saving children; daily researching thousand prescriptions may exhaust the wonder of benefiting others. However, discussing physical formation beginning, innate spirituality, unspoken language, this is the "Mute Department" (Pediatrics). Only a master of flexible wisdom can observe those syndromes. Must refine preparations to remove chronic ailments. Those who trust shamans instead of doctors are greatly mistaken; having formulas but not valuing herbs is even more erroneous. This is the patient's family taking blame themselves; doctors are helpless. Techniques displayed by Bian Que of Xianyang verified infant preservation; pulse clarity by Shu He of Jin Dynasty ensured no error in pediatrics. Furthermore, white complexion indicates excess deficiency, spirit is often dull; purple-black indicates excess, voice is louder. Yang syndromes resemble Yin, pulse feels fine and tight upon pressing; Yin syndromes resemble Yang, pulse comes slow and responsive. As the Great Learning says, "As if protecting an infant, sincerely seeking," this again requires investigating the mind and observing syndromes.
I once said that practicing medicine is not easy, and the pediatric specialty is especially difficult.
The Yellow Emperor said: "If I cannot perceive the condition of the young, even a sage would find it difficult, let alone later learners." Though I am untalented, I casually composed the previous fu. To facilitate learners in observing forms and taking pulses, at the time of illness, not only providing topics for conversation, but also displaying the wonders of divine, sacred, skillful, and adept medicine, is it not virtuous? If not so, let future wise ones correct and verify it; this is great fortune.
