Another interpretation: The Jueyin meridian contains two Yin lines and is responsible for storing essence and blood (its trigram image is Zhen). As long as one Yang line stirs, the four limbs remain healthy. If Jueyin damages one of its Yin lines (this transforms into a pathological Li trigram, indicating injury to essence and fluids, resulting in Xiao Ke [wasting-thirst syndrome], with Qi rushing upward to strike the heart, causing pain and heat in the chest, yet an inability or unwillingness to eat; its trigram image is Dui, because the middle Yang line of the Zhen trigram is filled with pathogenic heat), if that Yang line is further injured (transforming into a pathological Kun trigram), it will lead to incessant diarrhea and injury to the stomach and intestines.
In summary, the differentiation of the Six Meridians does not go beyond the two characters of Yin and Yang; the key lies in working upon Yin and Yang. Once Yin and Yang are clearly differentiated, treatment becomes easy as turning over a hand. Most difficult and chronic diseases today can be resolved effortlessly by combining them with the differentiation of the Six Meridians.
Chapter 4: Principles of Yin-Yang Differentiation
Section 1: Yin-Yang Differentiation 1
Golden Mantra of Yin-Yang Differentiation
Yin Syndrome
The lips and mouth are cyan and pale, the eyes lack spirit, the voice is low, breathing is short, and there is only a desire to sleep. The body feels heavy, speech is reluctant, and the body fears cold. Food has no taste, and clear fluid is vomited. The tongue body is cyan and slippery or black and moist. The coating is white, light yellow, and smooth. There is abundant saliva in the mouth, but no desire to drink. Although thirsty and desiring to drink, there is no desire to swallow, or perhaps a preference for hot drinks. Urine is clear and copious, and stool is loose. The breath has a fishy odor, and cold and heat symptoms are mixed, with more cold than heat and an inability to eat. The pulse lacks spirit; this is a Yin syndrome. Even if the tongue has no coating but the mouth is moist, and urination and defecation are normal without thirst, focusing on restoring Yang will ensure no errors.
Yang Syndrome
Due to Yin deficiency, the face, eyes, and lips are red. Spirit is unwearying, and there is insomnia. There is bad breath, coarse breathing, and a loud voice. There is aversion to heat, a light sensation in the body, and dry stool. Urine is dark yellow and scanty, with difficulty in urination. Thirst drives the patient to drink cold liquids continuously. All six pulse positions are large and forceful. The tongue coating is dry yellow or black-yellow, with no saliva in the mouth and a prickling sensation. There is irritability, tidal fever, or night sweats. There is dry cough with much or little phlegm. The pulse has spirit; this is a Yang syndrome. If there is thirst for cold drinks, body heat, and obstruction of urination and defecation accompanied by irritability and delirium, and the body is cold as ice resembling death, this indicates extreme heat lurking internally. Yang fails to reach the exterior, appearing as pure Yin. At this time, one must check the breath. Although the breath may be slight, it emits heat. The root of the tongue is red but not cyan. One should urgently use purgative methods to preserve Yin fluids. Do not mistake this for a Yin syndrome and endanger life.
Section 2: Decision on True and False Mixed Diseases of Yin-Cold and Yang-Heat
Yin manifests as cold, and Yang manifests as heat; this is Yin and Yang. The complexity lies in distinguishing true from false. True heat presents with thirst, yellow urine, spirit in the pulse, and irritability/restlessness. True cold presents with frequent urination, pale complexion, lack of spirit in the pulse, fatigue, and only a desire to sleep. True heat involves thirst and a preference for cold drinks; false heat involves rinsing the mouth without swallowing. True heat involves dry stool and a red face; false heat involves a red face but clear, copious urine. True heat involves coarse and long breathing; true cold involves cold, short, and weak breathing. True heat presents with a scorched tongue and dry yellow coating; true cold presents with a cyan tongue and watery slippery coating. Extreme Yin resembles Yang, known as Dai Yang (floating Yang); extreme Yang resembles Yin, with limbs icy cold. One must carefully weigh true and false cold and heat. Once Yin and Yang syndromes are clearly distinguished, true heat Yang syndromes are treated with bitter, sweet, and cold herbs, while true cold Yin syndromes are treated with warm decoctions. Understanding the mechanism of creation in Yin and Yang provides a way to treat difficult and miscellaneous diseases. The primordial establishment is Water and Fire; when Water and Fire are harmonized (Ji Ji), there are no chronic illnesses. Spleen Yang deficiency leads to loose stool, which subsequently harms the Liver. Hepatitis B virus takes advantage of this, and Fuzi (Aconite Lateralis Radix Praeparata) and Ganjiang (Zingiberis Rhizoma) are used. Kidney Yang deficiency leads to frequent urination, leakage of essence leading to dry eyes, dizziness, and lower back pain; Hu Yang Jiu Ni Si Ni Tang (Decoction for Restoring Yang and Saving from Collapse) is used. Leukemia is caused by Yin-cold pathogens penetrating the bone marrow, increasing white blood cells. Western medicine only knows how to kill white blood cells, thereby destroying Yang Qi. In TCM, this disease belongs to Shaoyin. For Taiyang-Shaoyin simultaneous infection, Mahuang Fuzi Xixin Tang (Ephedra, Prepared Aconite Root, and Asarum Decoction) is used; for Shaoyin deficiency-cold, Si Ni Tang (Frigid Extremities Decoction) is used. This method is simple, convenient, inexpensive, and effective, showing miraculous power. AIDS is a separation of Yin and Yang. Western medicine detects this through blood tests: normal blood shows one line, while AIDS blood shows two separated phases. When Yin is calm and Yang is secure, spirit is regulated; when Yin and Yang separate, spirit perishes. Chinese and Western medicine harmonize Yin and Yang, being simple, convenient, inexpensive, and highly effective.
Chengkong's Alternative Interpretation of Six Meridians and Eight Trigrams
The Eight Trigrams cover Heaven and Earth, making it easy to explain physical laws. Three Yao lines form a trigram, and there are eight pure trigrams, each composed of three lines.
Shaoyin has less Yin (Trigram Li): Two Yang lines embracing Yin constitute the Heart (Li). Two Yin lines embracing Yang constitute the Kidney (Kan). Shaoyin stores essence and fluids within Yin. If Shaoyin injures Yang, limbs cannot extend (due to reduced Yang), presenting with aversion to cold, curling up, and only a desire to sleep. Floating deficient Yang causes lesions at the root of the tongue, enlarged tonsils with fever, dizziness, head distension, hypertension, and cold extremities. If Kan Yang is injured and transforms into Kun (pure Yin), life becomes stagnant; who knows the cause? Si Ni Tang supports Kan Yang, leaving no place for Shaoyin cold transformation to hide. If Shaoyin injures Yin, damaging essence and fluids, irritability and thirst for cold drinks occur, with dark, scanty urine as the cause. Huanglian Ejiao Tang (Coptis and Donkey-Hide Gelatin Decoction) is decocted and taken.
Taiyang has much Yang (Three Yang lines, Trigram Qian, representing Strength): It strongly protects the body surface. In Taiyang disease, Yang is trapped, causing fever due to stagnation. If Spleen Yang is sufficient, there is no sweating; Majiang Tang (Ephedra Decoction) should be decocted urgently. Sweating disperses cold, and the exterior becomes healthy. If Spleen Yang is insufficient, pores are loose, causing aversion to wind, sweating, and deficiency of Ying and Wei. Guizhi Tang (Cinnamon Twig Decoction) resolves this. If Shaoyang Yang Qi is deficient, cold pathogens enter Shaoyin directly, forming a combined Taiyang-Shaoyang disease.
Yangming has two Yang lines (Trigram Li): Adding one Yang line (stagnant heat) injures Yangming (pathological Qian trigram). Yangming channel syndrome presents with irritability and thirst. In new diseases where stool is not yet solid, Baihu Tang (White Tiger Decoction) is effective. In chronic Yangming disease, "Stomach Home Excess" occurs, with dryness, excess, fullness, and oppression potentially appearing together; San Chengqi Tangs (Three Bowel-Purging Decoctions) are applied according to the syndrome.
Shaoyang has one Yang line (Trigram Kan): Essence juice is stored internally; excretion brings auspiciousness. Disease arises from dampness encumbrance and stagnant heat (Trigram Gen). If storage occurs without excretion, the Gallbladder suffers. Sanjiao blockage closes water pathways; failure of Juedu (drainage) function causes Shaoyang disease. Alternating chills and fever are treated with Xiaochaihu Tang (Minor Bupleurum Decoction). Taiyin dampness encumbrance requires drying dampness using Banxia (Pinelliae Rhizoma), Cangzhu (Atractylodis Rhizoma), and Yiyiren (Coicis Semen).
Taiyin has much Yin (Three Yin lines, Trigram Kun): Yin stores essence, Qi, and spirit. The Spleen is the official of transportation and transformation, relying on it to transport quintessence from grains. Excess dampness encumbers the Spleen, causing Taiyin disease, with abdominal fullness and cough as causes. Taiyin abdominal fullness with constipation is treated with Dahuang (Rhei Radix et Rhizoma), Fuzi (Prepared Aconite Lateralis Radix), and Xixin (Asari Radix et Rhizoma). Yang jaundice uses Yinchen Hao Tang (Artemisiae Scopariae Herba Decoction); Yin jaundice adds Baizhu (Atractylodis Macrocephalae Rhizoma). Taiyin Yin injury with constipation uses Lizhong Wan (Regulate the Middle Pill) with heavy Baizhu, plus Maidong (Ophiopogonis Radix) and Wuweizi (Schisandrae Fructus).
Jueyin has two Yin lines storing essence and blood (Trigram Zhen): One Yang line stirring ensures healthy limbs. If Jueyin injures one Yin line, Xiao Ke [wasting-thirst syndrome] occurs, Qi strikes the heart, causing chest pain/heat and anorexia. If the Yang line is further injured, incessant diarrhea injures the stomach and intestines. Jueyin diseases feature mixed cold and heat; treatment balances cold and heat based on Yin and Yang.
Alternative Interpretation of Jueyin: Jueyin is one Yang supporting two Yins (Trigram Zhen). Cold entering Jueyin injures one Yang. Jueyin and Taiyin are neighbors; Yang injury causes Spleen dysfunction, dampness encumbers Taiyin, Spleen Yang sinks, leading to Xiao Ke with excessive drinking and clear urine. Heat entering Jueyin injures one Yin, causing thirst, irritability, and delirium. Whether cold or heat enters Jueyin, mixing cold and heat results; Jiang (Ginger), Fu (Aconite), Jiao (Zanthoxyli Pericarpium), Bo (Phellodendri Cortex), and Wumei (Mume Fructus) are used.
Chapter 5: Chengkong's Golden Mantra for Yin-Yang Differentiation and Medication
Section 1
Chengkong distinguishes Yin and Yang in treating diseases, matching medication to Yin-Yang injury. Loose stool indicates Spleen Yang injury; Ganjiang (Dried Ginger) warms the middle and strengthens Spleen Yang. Thirst with clear, copious urine indicates injury to the single Yang in Kan (Kidney); fluids fail to ascend to the mouth and throat. Hu Yang Hua Qi Si Ni Tang (Decoction for Restoring Yang and Transforming Qi) is used. Si Ni Tang contains Fuzi (Prepared Aconite Lateralis Radix) and Ganjiang (Dried Ginger), assisted by Sheng Gancao (Raw Licorice Root) and Zhi Gancao (Honey-fried Licorice Root). Sheng Jiang (Fresh Ginger) is juicy, enters the Spleen, and its acrid-hot nature awakens the Spleen and dispels dampness. Hunger without desire to eat indicates Stomach Yin injury; Yiguan Jian (One Glance Infusion) is decocted. Appearance of dryness, excess, fullness, and oppression requires urgent purgation to preserve Yin. A fat tongue with teeth marks indicates dampness injuring the Spleen (Taiyin); Cangzhu (Atractylodis Rhizoma), Yiyiren (Coicis Semen), and Banxia (Pinelliae Rhizoma) dry dampness and strengthen the Spleen. The Lung is a delicate organ intolerant to cold; cold injuring the Lung causes continuous cough. Dispersing Lung cold is key; Majiang Tang (Ephedra Decoction) is decocted urgently. The Lung is also intolerant to heat; heat injuring the Lung causes yellow, thick phlegm. Clearing Lung heat is achieved with Maxing Shigan Tang (Ephedra, Apricot Kernel, Gypsum, and Licorice Decoction). Fever without aversion to cold is Taiyang disease; dispersing Lung cold uses Majiang Tang. Sweating with aversion to wind indicates Ying-Wei deficiency; Guizhi Tang (Cinnamon Twig Decoction) is decocted. No fever but aversion to cold is Shaoyin disease; Fuzi, Ganjiang, and Gancao form Si Ni Tang. If cough is present, Mahuang Fuzi Xixin Tang (Ephedra, Prepared Aconite Root, and Asarum Decoction) or Muxing Si Ni Tang is used.
Section 2
Preliminary Discussion on Medication for Six Meridians Yin-Yang Differentiation
Chengkong discusses herb properties via Yin and Yang: Acrid and sweet promote dispersion and are Yang; bitter, sweet, and sour are cold and Yin. Yin syndromes use Yang herbs; Yang syndromes use bitter, sweet, and cold herbs. Taiyin disease involves inability to eat, cough, and abdominal fullness. Taiyin damp-earth requires drying; Cangzhu (Atractylodis Rhizoma), Yiyiren (Coicis Semen), and Banxia (Pinelliae Rhizoma) are used. Heavy dampness with light heat uses Fuzi (Prepared Aconite Lateralis Radix) and Ganjiang (Dried Ginger); once dampness is gone, heat has nowhere to hide. Heat heavier than dampness uses Simiao San (Four Marvels Powder) to dispel dampness and clear heat. Yinchen Tang (Artemisiae Scopariae Herba Decoction) treats Yang jaundice; Yin jaundice adds Zhufu Tang (Atractylodes and Aconite Decoction). Shaoyin disease presents with only a desire to sleep, aversion to cold, curling up, and faint, fine pulse.
Shaoyin deficiency-cold uses Fuzi (Prepared Aconite Lateralis Radix) and Ganjiang (Dried Ginger), assisted by Sheng Gancao (Raw Licorice Root) and Zhi Gancao (Honey-fried Licorice Root). Taiyang-Shaoyin simultaneous infection uses Mahuang Fuzi Xixin Tang (Ephedra, Prepared Aconite Root, and Asarum Decoction); Shaoyin deficiency-cold uses Si Ni Tang (Frigid Extremities Decoction). Transformation to Yang heat with thirst, dryness, and irritability uses Huanglian Ejiao Tang (Coptis and Donkey-Hide Gelatin Decoction). Jueyin mixed cold and heat requires complex herbs: Fuzi (Prepared Aconite Lateralis Radix), Ganjiang (Dried Ginger), Jiao (Zanthoxyli Pericarpium), Lian (Coptidis Rhizoma), and Bo (Phellodendri Cortex). Taiyang cold-water requires warming; Guizhi (Cinnamon Twig), Mahuang (Ephedra), Ganjiang (Dried Ginger), and Dazao (Jujubae Fructus) are added. Fever without sweat is Taiyang disease; no fever but aversion to cold is Shaoyin disease. Aversion to wind with sweating uses Guizhi Tang (Cinnamon Twig Decoction); fever without sweat uses Majiang Tang (Ephedra Decoction). Taiyang water accumulation uses Wuling San (Five-Ling Powder); blood accumulation uses Taohexiaochengqi Tang (Peach Kernel and Major Order the Qi Decoction). Taiyang cold-water arises from Shaoyin deficiency-cold impairing Qi transformation. Shaoyang alternating chills and fever require Xiaochaihu Tang (Minor Bupleurum Decoction). Alternatively, Danggui Sini Tang (Angelica Sinensis Decoction for Cooling Extremities) boosts Yang to expel pathogens. Yangming disease "Stomach Home Excess" uses San Chengqi Tangs (Three Bowel-Purging Decoctions) according to the syndrome. Yangming channel syndrome with un-solidified stool uses Baihu Tang (White Tiger Decoction) to remove excess heat.
Chapter 6: Miscellaneous Discussions on TCM
TCM "Qi Transformation Theory," "Syndrome Recognition Theory," "Treatment Theory," "General Theory of Hundred Diseases"
The human body is a mass of Qi formed by Water and Fire, nourishing internal Zang organs and protecting the external body, reaching nine orifices and warming limbs. Yet people do not realize this, indulging in cold foods and drinks, damaging Yuan Qi. One or two days pose no major issue, but over time, Qi becomes weak. Cold-dampness attacks Taiyin Lung, causing cough and irregular breathing; it attacks Taiyin Spleen, causing loss of transportation, confusion of clear and turbid, and undigested diarrhea. Spleen Yang injury leads to loose stool, harming the Liver. Failure to retain essence leads to cirrhosis. Gastrointestinal mucosa is injured by cold and heat, eventually causing ulcers. Lack of nourishment to clear orifices causes various changes, including loss of vision and hearing. Avoiding grains and eating meat/fish accumulates sinews daily; lipid deposits block clear orifice pathways, leading to limb paralysis and staggering. Young men indulging in lust suffer Shaoyin deficiency-cold and essence emptiness. Spleen Yang depression causes essence leakage, triggering Xiao Ke [wasting-thirst syndrome]. Essence injury is the root of all diseases. Internal injury leads to external contraction; all diseases arise from Wind. The Nei Jing records Six Meridian syndromes; Shanghan Zabing Lun details them. Seven Emotions disrupt Qi; emotional diseases require emotional healing. Natural Six Qi nourish humans; excess or deficiency becomes pathogenic. Chengkong TCM starts from Qi theory, understanding all diseases. Medication distinguishes Yin and Yang; syndrome recognition does not depart from Yin and Yang. Yin-Yang differentiation is detailed, followed by Six Meridians and Zang-Fu. Syndromes classify diseases; Yin-Yang constitution is fundamental, disease is merely a mark. Reversing Yin and Yang hides disaster. Yin syndromes use warm-hot herbs; Yang syndromes use bitter-sweet-cold herbs. Mixed Yin-Yang uses cold-hot herbs; excess dampness requires hot herbs. Primordial establishment is Water and Fire; when harmonized, no chronic illness exists. Harmonized Water and Fire allows Qi transformation, ensuring sweet dreams. Unharmonized Water and Fire causes insomnia and chaotic spirit. Yi Jing transmits Yin and Yang; one Yin and one Yang are hidden in books. All diseases involve Yin and Yang; without Yin and Yang, there is no discourse. The sages' true transmission is one sentence; fools store ten thousand volumes. Without understanding Yin and Yang, do not discuss medicine, knowing it harms lives. If one understands this wonder, why entrust the precious body to mediocre doctors?
Theory of Six Meridians Qi Transformation
I. General Discussion:
All five Zang organs are born from Qi transformation; the functions of the six Fu organs are accomplished by Qi transformation. Heart and Kidney Qi transformation improves memory; opening and closing of urination and defecation connects all vessels. Six Meridian Qi transformation is stored in the five Zang organs; the six Fu organs transmit and drain, allowing turbid Qi to flow.
II. Specific Discussion:
1. Taiyang Small Intestine and Bladder:
Taiyang Qi transformation prevents cold invasion; its function is separating clear from turbid. Bladder Qi transformation excretes urine; if Qi transformation fails, urination is obstructed.
2. Yangming Stomach and Large Intestine:
Yangming Qi transformation generates Qi and Blood. The Stomach rots and ripens grains; Large Intestine Qi transformation generates fluids and excretes waste, preventing disease. Impaired Qi transformation causes fever, constipation, abdominal distension, and severe thirst.
3. Shaoyang Gallbladder and Sanjiao:
Shaoyang Qi transformation generates bile, which enters the Fu organs to decompose lipids. Impaired Qi transformation blocks bile, causing gallstones. Sanjiao Qi transformation harmonizes Zang organs. Impaired Qi transformation generates miscellaneous diseases.
4. Taiyin Spleen and Lung:
Taiyin (Lung and Spleen) Qi transformation ensures good respiration. Spleen Earth Qi transformation transports micro-essence; blood in vessels maintains Ying. Impaired Qi transformation causes blood extravasation, subcutaneous bleeding, gum bleeding, and menorrhagia. Lung Metal Qi transformation ensures smooth respiration, preventing cough. Nasal congestion is often due to cold blocking the Lung; impaired Qi transformation causes clear runny nose. Warming the Lung and dispersing cold strengthens the Spleen meridian. Cold stagnation transforming to heat causes yellow nasal discharge; over time, Bi Yuan (nasal sinusitis) develops. Treatment disperses stagnant heat, diffuses the Lung, transforms phlegm, and opens nasal orifices.
5. Shaoyin Heart and Kidney:
Heart and Kidney interaction achieves Qi transformation. Shaoyin Qi transformation connects all vessels. Fluids ascend to moisten the throat. Heart governs blood vessels; Kidney water is abundant. Kidney controls urination and defecation via Yang function. Heart-Kidney non-interaction causes insomnia; failed Qi transformation impedes all vessels.
6. Jueyin Liver and Pericardium:
Jueyin Qi transformation stores Liver blood, nourishing five Zang organs by day and hiding Hun by night. Pericardium meridian provides warmth, allowing Heart blood to circulate continuously. Loss of Qi transformation causes blood extravasation and vessel failure to store blood. Cold injuring the Pericardium causes blood to lose warmth, hindering Heart blood circulation.
Clinical Syndrome Discussion (Written Spontaneously)
Standards for Diagnosing and Treating Diseases
TCM itself is a matter of Yin and Yang. Anyone discussing standards apart from Yin and Yang has non-standard standards. Like when I heard a TCM doctoral supervisor at the China Academy of Chinese Medical Sciences say they wanted to establish medication standards for stroke, someone immediately asked, "Using your standard, what is the efficacy?" Indeed, TCM emphasizes efficacy, especially long-term efficacy. Can one indirectly treat diseases not yet occurring while treating the current disease? The answer is yes. Because an excellent TCM practitioner is a Yin-Yang master who can see the "past," "present," and "future" of a disease. These three correspond to the "San Cai" (Heaven, Earth, Human) discussed in Yi Jing. Some say TCM does not need to study Yi Jing. Is it possible without studying Yi Jing? Without understanding Yin and Yang, how can one discuss TCM? Without knowing San Cai, how can one discuss the human body? Do not forget that "Man" has Heaven above and Earth below, standing between Heaven and Earth. Heaven, Man, and Earth are San Cai. Is it acceptable not to understand Wu Yun Liu Qi (Six Qi and Five Movements)? Without understanding Wu Yun Liu Qi, how can one adapt to times? Is it acceptable not to understand Feng Shui? Feng Shui says, "Where mountains surround and waters embrace, there must be Qi." This Qi is Yang Qi, also called Sheng Qi (Vital Qi). When choosing living conditions, we first consider sunlight. Sun is Yang; acting on a location generates Yang Qi. Whether it stays depends on backing against a mountain. Diet is similar; eating too much cold damages Yang Qi. Once Yang Qi is damaged, all diseases arise. Therefore, the TCM standard is thoroughly understanding Yin and Yang. Thus, one must understand Yi Jing, knowing Jian Yi (simplicity: using simple formulas), Bu Yi (unchanging truth: breaking Yin cold, restoring Yang Qi, preserving Yin essence), and Bian Yi (change: adapting to time, place, and constitution). One must read Nei Jing, Shanghan Zabing Lun, and Shennong Ben Cao Jing thoroughly. Specifically, treating disease first distinguishes Yin and Yang syndromes, then determines which of the Six Meridians (containing all twelve meridians) it belongs to, and finally prescribes medication according to the meridian (warm-hot herbs for Yin syndromes, bitter-sweet-cold herbs for Yang syndromes). This is my method; efficacy can be seen in medical cases. Many clinical masters use this method. Do you think this standard is acceptable?
Laws of TCM:
The law of TCM is the waxing and waning of Yin and Yang. When Yang declines, Yin-cold (pathogens) grow, and disease increases; when Yang grows, Yin-cold (pathogens) decline, and disease recedes. Abundant Yin essence leads to abundant Yang Qi; abundant Yang Qi secures Yin essence. Yin essence is the root of Yang Qi and must not be deficient. Deficiency leads to winter cold injury and spring warm diseases.
Briefly on Pulse Diagnosis:
Pulse diagnosis is merely a skill, a lower-level method in TCM diagnosis. Ancients said: Knowing by observation is Divine; knowing by listening/smelling is Sage-like; knowing by questioning is Artisan-like; knowing by palpation is Skillful. Since many find pulse taking mysterious, I will briefly discuss it. Pulse diagnosis judges disease based on position, rate, shape, and momentum.
Pulse Position includes Cun, Guan, Chi, and Fu, Zhong, Chen (superficial, middle, deep). Cun views Heart, Lung, and Head; Guan views Spleen and Stomach; Chi views Liver, Kidney, legs, feet, and Bladder. Superficial pulse views Heart, Lung, Head; middle pulse views Spleen, Stomach; deep pulse views Liver, Kidney. Expert pulse readers do not strictly distinguish left and right hands.
Rate: The frequency of pulse beats. Calculated by the doctor's breath, four beats per inhalation-exhalation cycle (five occasionally) is Huan Mai (Moderate Pulse). A moderate pulse with strength on deep pressure is normal. Six beats per cycle is Shu Mai (Rapid Pulse); seven or more is Ji Mai (Hasty Pulse).
Shape: The form of the pulse.
Momentum: The strength of the pulse.
Then, twenty-eight pulses are governed by six basic pulses.
Six Pulses: Fu (Superficial), Chen (Deep), Chi (Slow), Shu (Rapid), Xu (Deficient), Shi (Excess).
Fu Category: Fu, Ru, Hong, Kou, Ge, San.
Chen Category: Chen, Fu, Lao, Ruo.
Chi Category: Chi, Huan, Se, Jie.
Shu Category: Shu, Cu, Ji, Dong, Dou.
Xu Category: Xu, Wei, Xi, Dai, Duan.
Shi Category: Shi, Hua, Jin, Chang, Xian.
Specific Rules: Use Fu/Chen to determine Exterior/Interior; Chi/Shu to determine Cold/Heat; True/False to determine Yang/Yin syndrome. Use Force/Weakness to determine Excess/Deficiency. In clinical practice, I mainly follow Nei Jing theory: "When Yin is calm and Yang is secure, spirit is regulated." I observe whether Cun, Guan, Chi are excessive or insufficient. Excess is Excess syndrome; insufficiency is Deficiency syndrome. But I seek moderation and root. Root means strong Chi pulse. Rapid with strength and root is Yang Excess-Heat; rapid without strength is Yin Essence Deficiency Deficiency syndrome; slow without strength and root is Deficiency-Cold Yin syndrome. I also check the temperature of Chi skin (inner wrist) and refer to urination/defecation to clarify Yin/Yang syndromes.
Example: A patient arrives; the physician must take the pulse. Otherwise, no matter how skilled, the patient won't trust. First impression is Chi skin temperature. If Chi skin is cold, next check Cun pulse. If Cun is rapid and stronger than Guan/Chi, one can diagnose: dizziness, head distension, palpitations, fatigue, heavy legs, even lower back pain. Patients usually say, "Accurate." Why? Cun rapid indicates heat above; heat causes head distension because Cun governs Head, Heart, Lung. Guan/Chi weakness indicates Spleen-Stomach transportation failure, Liver blood deficiency, and Kidney Yang deficiency. Here, Cun rapid is false, caused by Kidney Yang deficiency leading to floating deficient Yang. Other pulses follow this logic.
How to perceive the coming and going of the pulse?
