Grind into fine powder. Take 2 qian per dose, mixed with light radish soup.
Feng Luzhan stated: The nose is the orifice of the Lung. The Classics state: "Heavenly Qi connects with the Lung." If there is no phlegm-fire accumulation-heat in the intestines and stomach, then what normally rises is clear Qi. When the Lung house has disease, the nose becomes obstructed. For example, heat pathogen that does not dissipate, or cold damage that lingers and transforms into heat over time, can both cause obstruction and difficulty in breathing. If a healthy person has excessive nasal discharge, whether yellow or white, or bloody like pus, this is caused by Kidney deficiency. One must not overuse cooling medicines. (*Jinnang*)
He also said: Biyuan (Nasal Sinusitis/Brain Leak) refers to continuous dripping of nasal mucus, like a flowing spring, hence the name. The Classics state: "This is Gallbladder transferring heat to the Brain." The Gallbladder meridian originates at the outer canthus of the eye, ascends to the corner of the head, and enters/connects with the Brain. However, the Yangming meridian also flanks the nose, connects with the side of the eyes, and constrains the Taiyang meridian. Now, due to Brain heat, the Foot-Taiyang meridian rebels, becoming excessive along with the Yangming meridian, stagnating in the forehead. Thus, there is soreness and pain in the nose and forehead, and continuous dripping of nasal mucus.
He also said: Furthermore, if cold pathogen is not fully resolved and deficient heat gradually intensifies, purulent mucus accumulates, and the sense of smell is lost; this is called Biqiu (Nasal Obstruction/Loss of Smell).
He also said: Furthermore, if hot blood enters the face and is rebuffed by cold pathogen, turbid matter stagnates, forming Biqiu.
He also said: There are two types of Brain diseases: If clear water flows out without pain, it is Cold; if yellow, foul-smelling water flows out with pain, it is Heat. If prolonged and unhealed, it is named Naolou (Brain Leakage). Treatment should involve internal administration of herbs to clear and drain Gallbladder heat, and external moxibustion on the Xinhui and Tongtian acupoints.
He also said: Continuous flow of turbid nasal mucus is called Biyuan. This is caused by wind-heat scorching the Brain, causing fluids to seep downward. The discharge may be yellow or white, or bloody like pus; this is a sign of Kidney deficiency. The Classics state: "Brain seepage becomes mucus." It also states: "Gallbladder transfers heat to the Brain." *Yuanbing Shi* says: "Like using fire to smelt metal, extreme heat turns back into water." However, tracing the origin, it is inevitably due to Kidney Yin deficiency failing to grasp Qi and return it to its source. Thus, Fire acts without fear, pressing upward against Lung Metal. Consequently, the Qi of body fluids cannot descend, gathers in the orifices, transforms turbidity into mucus, and causes body fluids to flow in reverse. Thus, Kidney and Liver become increasingly deficient, with only ascending and no descending, only Yang and no Yin. Yin deficiency leads to disease; Yin exhaustion leads to death. One must guard against anger to nourish Yang, abstain from desires to nourish Yin, avoid fried foods, and stay away from alcohol and wheat products to prevent generating heat. Then, relying on a skilled physician, use nourishing Kidney and clearing Lung as the sovereign agents, resolving depression and smoothing Qi as the minister agents, and tonifying Yin and nourishing Blood as the assistant agents. This allows Fire to subside and Metal to clear, descending functions to proceed smoothly, Qi to flow freely and depression to resolve, clear orifices to be unobstructed, Yang opening and Yin closing to support each other, and Zang-Fu organs to perform their duties. By being cautious to cultivate the root and using medicine to treat the illness, occasional cures are possible. But if one is arrogant, indulgent, and careless, or mistakenly uses cooling medicines, even Cang Gong or Bian Que cannot grant longevity.
He also said: Long-term suffering from extremely foul-smelling thick nasal discharge is named Naolou. For Qi deficiency, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction); for Yin deficiency, use Maiwei Dihuang Tang (Lily and Cornus Pill with Rehmannia). The Classics state: "When Lung is harmonious, one can perceive smells." When Lung is congested and disharmonious, the nasal orifice is blocked, and one cannot smell.
Chen Shigong stated: Biyuan is also called Naolou. It is generally caused by cold and congealing into the Brain Gate, interacting with Sun-Yang damp-heat to generate steaming. The symptom is flowing turbid nasal mucus, or yellow water, dripping drop by drop, remaining wet for a long time without drying. Over time, dizziness and voidiness persist. Treatment is primarily Huoxiang Tang (Agastache Decoction). If dizziness persists due to long duration, internally administer Buzhong Yiqi Tang and Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) to nourish the source of transformation. (*Zhengzong*)
Chen Yuangong stated: Some people have spontaneous clear nasal discharge. After a long time, it becomes regular mucus; after even longer, it becomes yellow and turbid matter, like pus or marrow, with an unbearable fishy stench. Flowing for ten years leads to death. This results from excessive drinking and sleeping facing the wind. Wind enters the Gallbladder; the alcohol toxin in the Gallbladder cannot excrete outward, thus transferring heat to the Brain. The Brain receives the heat-toxin Qi and cannot store it long-term, so it exits through the nasal orifice. The Brain orifice connects with the nose, but how does Gallbladder Qi connect to the Brain, and how does alcohol Qi enter the Gallbladder? Those who drink well know that alcohol first enters the Gallbladder. The Gallbladder cannot withstand alcohol but can permeate it. Alcohol permeated by the Gallbladder dissipates its Qi. If one drinks too much, the Gallbladder cannot permeate it in time. Unable to permeate, it cannot overcome the alcohol; unable to transform the alcohol, fire-toxin remains inside. When lying down, Gallbladder Qi does not move, compounded by cold wind blowing, making the Gallbladder uncomfortable. Wood of the Gallbladder hates cold wind most. Internal cold invasion makes internal heat worse. Gallbladder belongs to Yang, and the Head also belongs to Yang. Gallbladder heat cannot stay in the Gallbladder long-term; it must transfer heat upward to the Head. The Brain is in the Head, and the Head has no place to store heat; it enters wherever there is an acupoint. Moreover, Gallbladder and Brain are originally connected. The Brain's acupoints are larger than those of the Gallbladder, so it settles there and does not return. Once settled in the Brain, movement prompts migration, seeking an orifice to exit, trending naturally toward the nose. Shallow fire-toxin yields clear mucus; deep fire-toxin yields turbid mucus. The longer it lasts, the heavier the flow. Eventually, mucus runs out, and brain marrow is entirely expelled; death is inevitable. Treatment method: Treating the Brain while still treating the Gallbladder is treating the source. Use Qiyuan Tang: Xinyi (2 qian), Danggui and Heishanzhi (Black Gardenia) (3 qian each), Chaihu and Beimu (1 qian each), Yuan Shen (1 liang). Decoct in water and take orally. Two doses reduce mucus; three doses cure completely. (Xinyi enters the Gallbladder, guiding Danggui to tonify Brain Qi, and Yuan Shen to resolve Brain Fire. Adding Chaihu and Zhizi soothes Gallbladder depression, so the Gallbladder does not assist Fire and naturally benefits from Qi tonification. Stopping nasal mucus by clearing Brain Fire and benefiting Brain Qi is the correct approach. The nose originally has no mucus; blocking the source upstream is better than damming the stream downstream! Some doubt using large amounts of Danggui, not knowing that when brain marrow is exhausted, large tonification is needed for Brain Qi to regenerate. Xinyi is dispersing and not for long-term use, so during its guiding phase, large doses of Danggui are used to tonify Brain and secrete essence, avoiding later use. If Xinyi is reduced later, heavy Danggui is useless. This is the key to sequential medication. People doubt Danggui due to its slippery nature harming the Spleen. Who knows that those with direct brain marrow leakage definitely fail to transform marrow into essence. Essence fails to transform, so it decreases. Decreased essence cannot distribute to the Large Intestine, causing dryness. Using Danggui to moisten is exactly what is desired; why worry?)
He also said: Some people have clear nasal discharge lasting years. People think it is internal heat causing Naolou, but who knows it is Lung Qi deficiency and Cold? Naolou is Biyuan. There are Cold and Heat syndromes, not just Gallbladder Heat. How to distinguish? Turbid and foul mucus indicates Heat; clear and odorless indicates Cold. Heat is Excess; Cold is Deficiency. Clear, odorless mucus is precisely Deficient-Cold. Heat requires cooling herbs; Cold requires warm-mild agents. If one universally uses dispersing methods without tonification, Lung Qi is damaged and Lung becomes colder, increasing clear discharge. Use Wenfei Zhiliu Dan: Renshen, Jingjie, Xixin (5 fen each), Hezi, Gancao (1 qian each), Jiegeng (3 qian). Decoct in water, mix with 5 qian of Shishouyu Naogu (Turbot Brain Bone) powder, and take orally. One dose stops the flow. (This formula is warm and mild, naturally warming the Lung, and possesses dispersing properties to expel pathogens, achieving miraculous results. Some argue Shishouyu Naogu was used by ancients for internal heat Biyuan, questioning how it treats Cold syndrome Biyuan, suspecting all Biyuan is Heat, not Cold. Not knowing Biyuan has Cold and Heat syndromes, and Shishouyu Naogu treats both. In Heat cases, mucus connects to the Brain; in Cold cases, mucus comes from the Lung. Medications enter the Lung, which are mild-warm. If the Lung is cold-cool, mild-warm resolves it. With Shishouyu Naogu blocking the path from the Brain, Brain Qi does not sink, and Lung Qi closes further, stopping the flow in one dose.)
He also said: Some people have nasal congestion and thick, sticky turbid mucus for years. People think it is Biyuan with Fire knotting in the Brain, but who knows it is Lung Depression-Fire not venting, not Biyuan. Depression syndromes exist in all five Zang organs, not just the Lung. *Neijing* states: "All Qi frustration and depression belong to the Lung." Lung depression means Qi blockage. *Nanjing* states: "Extreme Lung Heat causes mucus discharge." The Lung is a pure-empty organ, hating Heat most. Lung Heat makes Qi coarse and fluids rise, knotting into mucus. Extreme Heat makes mucus yellow; extreme Heat makes mucus turbid. This turbid matter cannot reside in the pure-empty organ and must exit via the nasal gate. Use Jiawei Xiaoyao San: Chaihu, Baizhu, Baifuling (2 qian each), Jiegeng, Baishao, Danggui (3 qian each), Huangqin, Banxia, Baizhi, Gancao (1 qian each), Chenpi (5 fen). Decoct in water and take orally. Two doses bring relief; eight doses cure completely. (This treats Liver Depression; why effective for Lung Depression? Because this formula treats Five Depressions, not just the Liver meridian. Added Jiegeng disperses Lung pathogens, Huangqin drains Lung Heat, and guides other ingredients into the Lung. How can depression not be vented? Thus congestion clears and turbidity dissolves.)
Li Dongyuan stated: Nasal congestion with loss of smell is commonly thought to be Lung Cold, but using pungent-warm resolving medicines is ineffective. Unknowingly, it is often due to pre-existing Fire pathogen in the Lung Meridian, causing blockage upon exposure to cold. Treatment should focus on clearing Lung and descending Fire, assisted by Qi-unblocking agents. If there was no previous history of congestion, but sudden onset due to cold-damp invasion causing stuffiness, heavy voice, or clear discharge, treat as Cold-Damp. However, in Qi-deficient persons, weak Qi cannot ascend, causing nasal stagnation. As stated, "Nine Orifices Blockage" arises from the intestines and stomach; frequent use of Buzhong Yiqi Tang will restore patency.
Zhu Danxi stated: The Lung opens into the nose; the Yangming Stomach Meridian also ascends flanking the nose. The Brain is the mansion of the Original Spirit; the nose is the orifice of the Life Gate. If Middle Qi is insufficient and Clear Yang does not ascend, the head tilts, and the Nine Orifices become unfavorable. The Classics state: "Heavenly Qi connects with the Lung." If there is no phlegm-fire accumulation-heat in the intestines and stomach, normal ascent is Clear Qi. Thus, the Twelve Meridians and 365 Collaterals send Qi and Blood upward to the face and through the orifices. Their Zong Qi (Gathering Qi) emerges from the nose to create smell. Yang Qi and Zong Qi are both generating Qi from the Stomach. If Spleen and Stomach are damaged by hunger, fullness, or labor, generating Qi weakens, and transporting Qi cannot ascend. Pathogens block the orifices, causing nasal disadvantage and inability to smell. Treatment should nourish the Stomach and solidify Ying Qi. When Yang Qi and Zong Qi ascend, patency returns. However, *Nanjing* says: "The Heart governs the Five Odors; the Lung governs all Qi." The nose is the Lung orifice, yet it perceives smells. Why? Speaking of the orifice, it is the Lung; speaking of function, it is the Heart. If Wei Qi (Defensive Qi) fails to guard, and cold pathogen lodges in the head, affecting the nose, it cannot function, leading to loss of smell. The Classics state: "Five Qi enter the nose and store in the Heart and Lung. Disease in Heart and Lung causes nasal disadvantage." Vision and hearing are bright and cool; distinguishing smells is warm. Treatment should first disperse cold pathogen, then tonify Stomach Qi, allowing Heart and Lung Qi to communicate, resulting in nasal patency and perception of smells.
