1. Male, surnamed Fang. After suffering from a severe case of Shanghan (Cold Damage disease), he frequently experiences palpitations and shortness of breath so severe that he cannot lie flat to sleep. His tongue is red, and his pulse is fine and rapid. Western medical fluoroscopy previously diagnosed cardiomegaly. Prescription: Korean Ginseng 15g, Prepared Rehmannia Root 30g, Cornus Officinalis Fruit 15g, Cinnamon Twig (An Gui) 3g, Gecko Tail 1 pair, Atractylodes Macrocephala Rhizome 15g, Schisandra Berry 6g, Agrimony Herb 30g, Calcined Oyster Shell 30g, Ophiopogon Tuber 15g, White Peony Root 15g. All ingredients are ground into a fine powder, mixed with refined honey to form pills the size of a walnut seed. Take 9g morning and evening.
[Commentary] After a severe illness, on one hand, Qi and Blood are damaged, and Blood fails to nourish the Heart, leading to palpitations and fright; on the other hand, Kidney Yin is depleted, causing deficient fire to flare upward, resulting in a red tongue and fine-rapid pulse; the Kidney fails to grasp Qi, leading to shortness of breath and inability to lie flat. The combination of Ginseng and Gecko is known as Shen Ge San (Ginseng and Gecko Powder), which has significant efficacy for asthma caused by the Kidney's failure to grasp Qi; combined with Rehmannia, Cornus, Oyster Shell, and Schisandra to greatly nourish Kidney Yin and grasp Kidney Qi; Ginseng, Ophiopogon, and Schisandra form Sheng Mai San (Generate the Pulse Powder), which can tonify the Heart Ying (Nutrient Level), benefit Qi and Yin; a small amount of Cinnamon Twig is used to guide fire back to its source and warm the Middle Jiao, preventing the cloying nature of the tonic herbs.
2. Male, surnamed Jin. The main symptom is aphonia (loss of voice), which developed gradually. He has afternoon fever, dry throat, red tongue, and fine-rapid pulse. There is strong suspicion of a consumptive disease (Sunken Disease), which is very different from ordinary cases caused by colds. Prescription: Scrophularia Root 12g, Ophiopogon Tuber 9g, Prepared Turtle Shell 18g, Artemisia Annua Herb 9g, Donkey-hide Gelatin Beads 15g, Bark of Groundcherry Root 12g, Mulberry Root Bark 9g, Dry Rehmannia Root 15g, Pig Skin Square (remove hair and meat, boil soup to replace water). [Commentary] Afternoon fever indicates Yin deficiency with fire hyperactivity; dry throat and red tongue indicate fluid depletion failing to ascend and moisten. Thus, it is clear that his aphonia belongs to a consumptive disease. The formula uses Rehmannia, Scrophularia, Ophiopogon, Donkey-hide Gelatin, and Pig Skin to increase fluids and moisten dryness; the Lung is a delicate organ most afraid of fire scorching, so Turtle Shell, Mulberry Root Bark, and Groundcherry Bark are added to purge fire and clear the Lung.
3. Female, surnamed Di. Symptoms include dizziness, palpitations, poor appetite, and difficulty sleeping soundly at night; her deficiency is increasingly severe. Treatment focuses on the Heart and Spleen. Prescription: Raw Astragalus Root 6g, Luodong Codonopsis Root 9g, Raw Atractylodes Macrocephala Rhizome 9g, Angelica Sinensis Root 9g, Poria Mushroom 9g (inferred from context), Fried Sour Jujube Seed 9g, Polygala Root 6g, Longan Aril 9g, Aucklandia Root 2.4g, Honey-fried Licorice Root 2.4g. [Commentary] This case belongs to Heart Blood deficiency. Gui Pi Tang (Restore the Spleen Decoction) can both tonify Blood and calm the Spirit, and also benefit Spleen Qi to aid the source of transformation, making it an excellent formula for treating simultaneous Heart and Spleen diseases.
4. Male, surnamed Huang. Dizziness has persisted for a long time, and his gaze lacks spirit. This is a sign of Liver and Kidney insufficiency. Prescription: White Peony Root 9g, Cuscuta Seed 9g, Lycium Fruit 9g, Vigna Umbellata Seed Coat 12g, Cornus Officinalis Fruit 9g, Fried Sour Jujube Seed 9g, Ilex Aquifolium Fruit 9g, Mulberry Fruit 12g, Black Sesame Seed 9g. Second Visit: After taking medicine, dizziness no longer occurs, but vision is blurred. The Kidney opens into the eyes; eye-brightening herbs are essentially soft-moistening Kidney-tonifying herbs. Prescription: Dry Rehmannia Root 18g, Cuscuta Seed 9g, Cuscuta Seed Cake 9g, White Peony Root 9g, Lycium Fruit 9g, Schisandra Berry 4.5g, Ilex Aquifolium Fruit 9g, Winter Mulberry Leaf 4.5g, Celosia Seed 9g, Black Soybean 18g, Black Sesame Seed 9g.
[Commentary] Although the Liver opens into the eyes, the Kidney stores Essence, and the essence of all Zang-Fu organs ascends to the eyes, hence it is said "the Kidney寄 (resides/opens) into the eyes." This patient's dizziness, lack of spirit in the eyes, and blurred vision are signs of insufficient Liver and Kidney essence and Qi, so medication focuses on softening and moistening Liver and Kidney Yin.
5. Male, surnamed Han. Lack of spirit, unable to sustain work, frequent dreams during sleep, chronic constipation; occasionally experiences leg cramps while walking, feeling like falling; occasionally loses coherence in speech for 1-2 minutes when talking; suddenly loses train of thought when writing but continues writing involuntarily, making it seem as if two hands wrote the article. The disease lies in Liver and Kidney insufficiency. Prescription: Raw Astragalus Root 12g, Lycium Fruit 9g, Codonopsis Root 6g, Cuscuta Seed 9g, Angelica Sinensis Root 9g, White Peony Root 9g, Coix Seed 18g, Hetuo Da Zao Wan (Heche Pill) 9g (swallowed in 2 doses). Second Visit: Has bowel movements daily but not smooth, appetite is poor. Prescription: Angelica Sinensis Root 9g, White Peony Root 9g, Prepared Fleeceflower Root 18g, Hemp Seed 9g, Sour Jujube Seed 9g, Biota Seed 12g, Quince Fruit 9g, Tender Mulberry Branch 12g. Additionally: Er Dong Gao (Two Winters Paste), Mulberry Paste, Astragalus Paste, each 18g, mixed, take 1 tablespoon morning and evening with boiling water.
[Commentary] The Brain is the Sea of Marrow; the Kidney stores Essence, generates Marrow, and connects to the Brain. Kidney insufficiency leads to an empty Sea of Marrow; the Liver stores Blood and governs the Tendons. Liver Blood deficiency leads to tendons losing nourishment. Therefore, the transient consciousness disorders and leg cramps appearing in this condition are attributed to Liver and Kidney insufficiency, and treatment begins with tonifying Liver and Kidney.
6. Female patient (referred to as Granddaughter). Sallow complexion, pale nails, indicating Qi deficiency and Blood insufficiency, with continuous night fever. Prescribed Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) with modifications. Prescription: Codonopsis Root 9g, Astragalus Root 15g, Atractylodes Macrocephala Rhizome 9g, Whole Angelica Sinensis Root 9g, Bupleurum Root 4.5g, Cimicifuga Rhizome 3g, Tangerine Peel 3g, Licorice Root 3g, Amomum Fruit 2.4g (added later), Eupatorium Leaf 9g, Cinnamon Bark Powder 1.0g (swallowed in 3 doses). Second Visit: After taking Bu Zhong Yi Qi Tang, body heat subsided. Qi deficiency is difficult to recover from quickly. Coating is greasy, mouth tastes bland. This prescription stimulates appetite. Prescription: Eupatorium Stem 9g, Amomum Fruit 2.4g (added later), Shallot Bulb 6g, Quince Fruit 9g, Chicken Gizzard Lining 9g, Finger Citron Slice 4.5g, Tangerine Peel 4.5g, Fried Barley Sprout 9g, Medicated Leaven 9g, Hawthorn Flesh 12g.
Third Visit: Inability to control bowels, involuntary defecation when urgent, indicating Spleen Yang deficiency. Prescribed Fu Zi Li Zhong Wan (Aconite Pill to Regulate the Middle). Prescription: Processed Aconite Chunk 4.5g, Earth-fried Codonopsis Root 9g, Fried Atractylodes Macrocephala Rhizome 9g, Charred Ginger 4.5g, Clear Honey-fried Licorice Root 3g, Asarum Root 2.4g, Schisandra Berry 4.5g, Honey-fried Aster Flower 9g, Honey-fried Polygala Root 4.5g. Fourth Visit: Loose stools and involuntary defecation are now controlled, but cough is severe with sticky phlegm. Prescription: Perilla Seed 12g, Inula Flower 9g (wrapped), Honey-fried Aster Flower 9g, Tangerine Peel 4.5g, Asarum Root 2.4g, Schisandra Berry 4.5g, Honey-fried Farfera Flower 9g, White Walnut Kernel 10 pieces (shelled), Honey-fried Polygala Root 4.5g, White Peony Root 9g, Clear Honey-fried Licorice Root 3g.
[Commentary] This is fatigue-induced fever, discussed in detail by Li Dongyuan in *Nei Wai Shang Bian Huo Lun* (Differentiation of Internal and External Injury). Using Bu Zhong Yi Qi Tang to remove fever with sweet-warm herbs matched the symptoms and produced immediate effect. The third visit's inability to control bowels was due to Spleen Yang deficiency, making Fu Zi Li Zhong Wan appropriate. Spleen Yang deficiency leads to failed transportation and transformation, accumulating fluids into phlegm, causing cough. The fourth visit treats the acute symptoms first using phlegm-resolving and Qi-descending herbs. For follow-up care, Xiang Sha Liu Jun Zi Tang (Aucklandia and Amomum Six Gentlemen Decoction) could be used.
7. Male, surnamed Dai. Dizziness, blurred vision, chronic insomnia, frequent erections; frequent nocturnal emissions, ranging from once a day to once every three to four months; red tongue, wiry-fine pulse. Treated as Yin deficiency. Prescription: Ilex Aquifolium Fruit 18g, Tribulus Fruit 18g, Eclipta Herb 12g, Vigna Umbellata Seed Coat 18g, Schisandra Berry 4.5g, Millet 12g, Fried Sour Jujube Seed 12g. Additionally: Da Bu Yin Wan (Great Supplement Yin Pill) 45g, 6g per dose, taken morning and evening.
Second Visit: Reports symptoms reduced by 70-80%, but feels feverish after labor, exhausted and weak, nocturnal emissions recur, and panting occurs after excessive walking. Should simultaneously tonify Qi and Yin. Prescription: Luodong Codonopsis Root 9g, Ophiopogon Tuber 12g, Schisandra Berry 4.5g, Vigna Umbellata Seed Coat 18g, Sour Jujube Seed 18g, Licorice Root 3g, Da Bu Yin Wan 24g. Third Visit: Sleep is acceptable, only many dreams, erection symptoms halved, nocturnal emissions about once every 20 days, recently feels heat rising with slight activity. Prescription: Raw and Prepared Rehmannia Roots 15g each, Dendrobium Stem 9g, Cuscuta Seed 12g, Cherokee Rose Fruit 18g, Raspberry 12g, Ophiopogon Tuber 9g.
[Commentary] Yin deficiency with fire hyperactivity causes insomnia and nocturnal emission; frequent emissions further injure Yin essence, intensifying deficient fire. The formula uses Danxi's Da Bu Yin Wan (Rehmannia, Anemarrhena, Phellodendron, Tortoise Shell, Pig Spinal Cord) and Er Zhi Wan (Two Solstices Pill) to nourish Yin and descend fire; Cuscuta and Schisandra to converge Kidney Qi; Sour Jujube Seed and Millet to calm the Spirit and soothe the Heart. The second visit shows fever after labor, indicating Qi and Yin dual deficiency with Yin failing to contain Yang, adding Sheng Mai San to tonify Qi and Yin. The third visit moistens Liver and Kidney, combining convergence to secure essence.
8. Female, surnamed Chen. Pulse of a cold is usually floating-tight; now it is rapid but fine and soft: fine indicates material deficiency, soft indicates functional decline. Yin and Blood levels refer to materials; Qi level refers to spiritual function. Medication for this constitution is too light and thus ineffective. Light herbs can dispel excess, but this cannot be applied uniformly. Prescription: Cassia Twig 2.4g (added later), Tribulus Fruit 9g, Saposhnikovia Root 6g, Vinegar-processed Bupleurum Root 4.5g, Gentian Macrophylla Root 9g, Licorice Root 2.4g, Ligusticum Rhizome 3.6g, Angelica Dahurica Root 6g, Medicated Leaven 9g, Eupatorium Stem 6g, Perilla Seed 9g (wrapped).
Second Visit: Ancient texts' concept of deficient heat may hypothetically correspond to parasympathetic nerve inhibition and sympathetic nerve excitation. Deficient heat is usually not high-grade, accompanied by occasional chills and fine-rapid pulses. Using tonic herbs for deficient heat merely restores normal function even if inhibited. Prescription: Cimicifuga Rhizome 2.4g, Angelica Sinensis Root 9g, Astragalus Root 9g, Bupleurum Root 4.5g, Raw Atractylodes Macrocephala Rhizome 9g, Cassia Twig 3g (added later), Codonopsis Root 9g, Tangerine Peel 6g, Honey-fried Licorice Root 3g, Poria Mushroom 12g, Fresh Ginger 3 slices.
[Commentary] The master elucidated and bridged the pathogenesis of deficient heat using both Traditional Chinese Medicine and Western medical theories. This type of deficient heat resembles functional low-grade fever; if persistent, Sweet-Warm Fever-Removing or Nourishing-Yin Clearing-Heat methods are commonly used. This case modifies Bu Zhong Yi Qi Tang to remove fever with sweet-warm herbs.
9. Male patient. Tongue is glossy and red; ancients considered this Yin injury. The meaning of Yin refers to all substances in the human body; Yin-nourishing herbs replenish these substances. Adding warming herbs excites gastrointestinal function. 14. Female, surnamed Chen. Chills followed by fever in the afternoon; once febrile, it persists until dawn. This is internal injury heat, hence the pulse is fine and rapid. Pulse characteristics and fever patterns are stimuli from pathogens; tonics aim to make pathogens self-destruct. Prescription: Angelica Sinensis Root 9g, Tuoli Cao (Agrimony Herb) 15g, Gentian Macrophylla Root 9g, Black Soybean 12g, Cimicifuga Rhizome 3g, Vinegar-fried Bupleurum Root 4.5g, White Peony Root 9g, Prepared Turtle Shell 24g, Fresh Ginger 3 slices, Jujube 7 pieces, Honey-fried Licorice Root 3g. [Commentary] Tuoli Cao is Agrimony Herb.

Prescription: Codonopsis Root 12g, Lycium Fruit 12g, Poria Mushroom 12g, Prepared Rehmannia Root 15g, Astragalus Root 18g, Atractylodes Macrocephala Rhizome 9g, Angelica Sinensis Root 15g, Red Jujube 7 pieces.
16. Male, surnamed Li. Anterior tongue is glossy red and slippery, trembling incessantly. Qing dynasty physicians proposed the concept of dual Qi and Yin injury. 19. Male, surnamed Liu. Causes of fever arising at specific times are broadly three: ① Seasonal disease Yangming syndrome, excess pattern; ② Anemia, what ancients called Yin-deficient heat; ③ If chills precede fever, this is the initial stage of tidal fever. Now malar flush appears intermittently, pulses are fine and rapid, indicating a consumptive disease. Prescription: Chinese Yam 9g, Silver Bupleurum Root 6g, Tender White Anemone Root 9g, Whole Angelica Sinensis Root 9g (oil removed), Euryale Seed 9g, Black Soybean 12g, Gentian Macrophylla Root 9g, White Peony Root 6g, Poria Mushroom 12g, Fragrant Tea Bud 9g, Prepared Pinellia Tuber 12g, Northern Millet 12g (wrapped).

Prescription: Cassia Twig 4.5g, White Peony Root 9g, Clear Honey-fried Licorice Root 4.5g, Green Dragon Bone 9g, Calcined Oyster Shell 24g, Cinnabar-coated Poria Mushroom 9g, Honey-fried Polygala Root 4.5g, Biota Seed 9g, Fried Sour Jujube Seed 15g, Pearl Mother 15g (crushed and decocted). [Commentary] Guizhi Tang can "harmonize Ying and Wei" for external injuries, and "transform Qi and regulate Yin and Yang" for internal injuries. This person is constitutionally weak; fever and spontaneous sweating indicate disharmony of Ying and Wei; frequent dreams indicate unsettled Heart Spirit. Hence, Guizhi Jia Longgu Muli Tang is used, adding herbs to calm the Spirit and soothe the Heart.
