After recent childbirth, Ying blood (nutritive blood) is severely depleted. Yin fluids are suddenly deficient in the lower jiao, and Yang qi easily floats upward. In severe cases, Qi collapse and blood fainting occur; if not treated promptly, it is incurable. In milder cases, head sweating and dizziness occur, known as Yu Mao (depression and vertigo).
When wind pathogen invades the sinews causing stiffness, it is called Fa Jing (spasm). When Yin deficiency leads to floating Yang causing fever, or when deficient fire flares up and scorches Metal (Lung), it causes wheezing, coughing, and shortness of breath. It may also manifest as empty vexation with insomnia, palpitations, and night sweats, eventually forming Ru Lao (postpartum consumptive disease). These are all signs of Yin deficiency and Yang hyperactivity. *Jin Gui Yao Lue* discusses three postpartum syndromes: First, blood deficiency with excessive sweating makes one susceptible to wind, leading to spasm. Second, loss of blood followed by sweating with excess cold leads to Yu Mao. Third, depletion of body fluids causes dryness in the Stomach, leading to difficult defecation. *Yi Zong Bi Du* states: Blood deficiency and sweating cause sinews and vessels to lose nourishment; wind invasion exacerbates spasms, which is a disorder of the sinews. Loss of Yin and blood deficiency causes Yang to rebel, and cold pathogen stagnates within, causing dizziness and blurred vision, which is a disorder of the Shen (Spirit). The Stomach stores body fluids to moisten the Yang channels. Depletion of fluids and Stomach dryness causes the Large Intestine to lose moisture, resulting in difficult defecation, which is a disorder of fluids. Although these three syndromes differ, they all result from loss of blood and injury to fluids. Therefore, medications should avoid acrid-hot substances that further deplete Yin. Even if there are external pathogens, one must avoid acrid-wind herbs that raise Yang, which could lead to death from excessive sweating and blood fainting.
Postpartum Yin injury inevitably damages the lower jiao. The Eight Extraordinary Vessels are mostly located in the lower jiao; Chong, Ren, Du, and Dai vessels lose their control. The most common symptoms include rebellious Qi attacking upward, lower back and spine pain, abdominal pain, and vaginal bleeding (red and white lochia). Mr. Xiangyan (Ye Tianshi) used Zi Shi Ying (Hematite) to subdue rebellious Qi for Chong vessel disorders. For Ren vessel disorders, he used Gui Ban (Tortoise Shell) to calm and secure. For Du vessel disorders, he used Lu Jiao Jiao (Deer Antler Glue) and Lu Rong (Deer Antler Horn) to warm and nourish. For Dai vessel disorders, he used Dang Gui (Angelica Sinensis) to tonify and circulate. For Yang Wei vessel disorders characterized by alternating chills and fever, he used Dang Gui Gui Zhi Tang (Angelica Twig Decoction) to harmonize Ying and Wei. For Yin Wei vessel disorders characterized by heart pain, he used Sheng Hua Tang (Generate the New Decoction) plus Rou Gui (Cinnamon Bark) to warm and dispel cold. These are treatments to be carefully applied based on syndrome differentiation in postpartum conditions.
In diagnosing new postpartum patients, first ask about abdominal pain to verify the presence of lochia. If there is distending pain in the lower abdomen, lochia is not cleared; use Yu Jin (Turmeric Tuber), Tao Ren (Peach Seed), Niu Xi (Achyranthes Root), and Yan Hu Suo (Corydalis Rhizome) to promote circulation. If a mass can be felt in the umbilical and abdominal area, it is an unresloved Er Zhen (Child Pillow/uterine hematoma); use Shi Xiao San (Quick Relief Powder) or Shan Zha (Hawthorn Fruit) and brown sugar to resolve and guide out stagnation. Abdominal pain relieved by warm pressure indicates deficiency-cold and Qi stagnation; use Sha Ren (Cardamom Seed), Mu Xiang (Aucklandia Root), Xiang Fu (Cyperus Rhizome), Xiao Hui Xiang (Fennel Fruit), Dang Gui, Sheng Jiang (Fresh Ginger), and Da Zao (Jujube) to warm and unblock. Next, inquire about headache and body pain, and whether there are chills/fever or sweating/no sweating, to distinguish between external and internal causes. For external contraction headache, the pulse is typically floating; use Xiong (Ligusticum), Bai Zhi (Angelica Dahurica), Fang Feng (Saposhnikovia), Jie Geng (not specified, likely Jing Jie), Gan Ju (Chrysanthemum), and Man Jing Zi (Vernonia Fruit) to disperse with acrid flavors. For blood deficiency headache, the pulse is rapid; primarily use Si Wu Tang (Four Substance Decoction) with Jiu Chao Bai Shao (Wine-fried White Peony Root). Postpartum colds should not be treated with heavy diaphoresis. If there is headache, fever, and floating pulse indicating wind-cold, use Xiang Su Yin (Perilla Infusion) plus Xiong, Dang Gui, Sheng Jiang, and Cong Bai (Scallion White) for mild release. Body pain with stiffness, aversion to cold, and tight pulse indicates cold pathogen; use Xiong Su Yin (Ligusticum Perilla Infusion) plus Sheng Jiang to warm and disperse. Initial dizziness and fever followed by irritability and thirst with a large right-sided pulse indicate warm-heat syndrome; use Cong Chi Tang (Scallion and Fermented Soybean Decoction) for mild sweating. If the pulse is slow with body pain, it is Ying deficiency; use Dang Gui Jian Zhong Tang (Angelica Build the Middle Decoction) to harmonize. Body pain after sweating indicates both Ying and Wei deficiency; use Gui Huang Jian Zhong Tang (Angelica and Astragalus Build the Middle Decoction) to harmonize both. Spontaneous sweating belongs to Yang deficiency; excessive sweating leads to Yang collapse. Mild cases use Ren Shen (Ginseng), Shu Di (Rehmannia), Bai Shao, Wu Wei Zi (Schisandra), and Xiao Mai (Wheat) to tonify and astringe. Severe cases use Shen Fu Tang (Ginseng and Aconite Decoction) to secure Yang. Night sweats belong to Yin deficiency; sweat is Yin fluid. Use Liu Wei Di Huang Wan (Six Ingredient Pill with Rehmannia) plus Mai Dong (Ophiopogon), Wu Wei Zi, and Mu Li (Oyster Shell) to secure Yin. Postpartum head sweating and syncope indicate Yang floating upward; use Sheng Di (Rehmannia), E Jiao (Donkey-hide Gelatin), Long Gu (Dragon Bone), Mu Li, Fu Shen (Poria with Hostwood), Suan Zao Ren (Zizyphus Seed), Wu Mei (Mume Fruit), Bai Shao, and Xiao Mai to nourish Yin and anchor Yang. Postpartum aversion to cold indicates insufficient Yang; if the Cun pulse is faint, use Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) plus Sheng Jiang and Da Zao to diffuse Yang. Postpartum fever indicates insufficient Yin; if the Chi pulse is wiry, use Liu Wei Di Huang Wan plus Rou Gui to gather Yin. Mutual contention of Yin and Yang causing aversion to cold and fever is treated with Ba Zhen Tang (Eight Treasure Decoction) or Shi Quan Da Bu Tang (Complete Great Tonifying Decoction) for regulation and tonification. If lochia is not cleared, use Da Tiao Jing San (Great Regulating Menses Powder) to reduce and tonify. If blood loss is excessive with intermittent fever, it is solitary Yang without attachment; use Si Wu Tang to benefit Yin, combined with Pao Jiang (Roasted Ginger) to gather floating Yang. If skin burns, face is red, thirst is present, and pulse is empty and large, use Dang Gui Bu Xue Tang (Angelica Tonify Qi Decoction) to generate有形 blood from formless Qi. Chills, fever, cough, emaciation, and sallow complexion indicate Ru Lao; treat with Mother Hen Soup, Pig Kidney Soup, or Ren Shen, Huang Qi, Fu Ling, Gan Cao, Wu Wei Zi, Shan Yao, Suan Zao Ren, Dang Gui, Bai Shao, and Lian Zi to support the Spleen. Cold abdomen and pain indicate lower jiao deficiency-cold; use Gui Jiang Yang Rou Tang (Angelica, Ginger, and Mutton Decoction) to warm and nourish. Excessive leukorrhea and lower back pain indicate Du vessel emptiness; fill with Lu Jiao Jiao, Gou Qi Zi (Goji Berry), Du Zhong (Eucommia), Sha Yuan Zi (Astragalus Complanatus), Tu Si Zi (Cuscuta), and Qian Shi (Euryale Seed). Ru Lao with chills/fever, poor appetite, and diarrhea indicates damage to Spleen Yang; use Yi Gong San (Extraordinary Skill Powder) plus Sha Ren, Lian Zi, Shan Yao, Yi Zhi Ren (Alpinia Oxyphylla), Rou Dou Kou (Nutmeg), and He Zi (Chebula) to warm and secure. Vomiting, diarrhea, and cold limbs indicate Stomach Yang deficiency; urgently warm with Fu Zi Li Zhong Tang (Aconite Regulate the Middle Decoction) or Wen Wei Yin (Warm Stomach Infusion). Vomiting, fullness, and excessive phlegm indicate Spleen Qi stagnation; use Xiang Sha Liu Jun Zi Tang (Aucklandia and Cardamom Six Gentlemen Decoction) to strengthen and transport. External contraction cough with heavy voice and nasal congestion indicates loose interstices; initially disperse with Xing Ren (Apricot Seed), Jie Geng (Platycodon), Su Ye (Perilla Leaf), Qian Hu (Peucedanum), and Sheng Jiang, then use Yi Gong San minus Bai Zhu plus Sheng Huang Qi (Raw Astragalus) to solidify. Internal injury cough with Spleen/Lung Qi deficiency and phlegm is treated primarily with Liu Jun Zi Tang plus Mi Zhi Sang Bai Pi (Honey-fried Mulberry Root Bark). Fire scorching Lung causing harsh cough is treated with Liu Wei Di Huang Wan minus Shan Zhu Yu and Ze Xie, plus Mai Dong and Wu Wei Zi. Dry cough without phlegm indicates fire郁 in the Lung; use Gan Jie Tang (Licorice and Platycodon Decoction) plus Yu Zhu (Polygonatum Odoratum), Bei Mu (Fritillaria), Xing Ren, and Bai He (Lily Bulb) to open and moisten. Postpartum blood loss with asthma is solitary Yang and absolute Yin, a critical condition; treat primarily with Zhen Yuan Yin (Source Securing Infusion). Rapid inhalation, spontaneous sweating, and cold limbs indicate vacuous Yang about to collapse; urgently rescue with Shen Fu Tang. If wind-cold external pathogen enters the Lung causing asthma, there will be coarse breathing and productive cough, differing from rapid shallow breathing; treat primarily with Jin Shui Liu Jun Jian (Metal-Water Six Gentlemen Decoction). Alternatively, remove Shu Di and add Xing Ren, Jie Geng, and Su Ye to disperse phlegm and regulate Qi. If retained blood rushing to the Lung causes asthma, use Ren Shen Su Mu Jian (Ginseng and Dalbergia Decoction) or Duo Ming San (Life-Saving Powder). Postpartum abdominal fullness, vomiting, and retained blood in the epigastrium are suitable for Di Sheng Tang (Rescue Sage Decoction). Food stagnation is resolved with He Zhong Yin (Harmonize Middle Infusion). Spleen Qi deficiency-cold is warmed and tonified with Liu Jun Zi Tang plus Pao Jiang and Mu Xiang. Stomach deficiency with rebellious Qi is bitter-descended with Ju Hong Ban Xia Tang (Tangerine Peel and Pinellia Decoction) plus Gua Lou (Trichosanthes) and Xing Ren. Liver Wood insulting Earth is smoothed with Liu Jun Zi Tang plus Sheng Ma (Cimicifuga) and Chai Hu (Bupleurum). Stomach deficiency hiccups is a critical symptom; use Li Zhong Tang plus Ding Xiang (Clove). Ancient method uses Ding Xiang San; if ineffective, urgently add Ren Shen and Fu Zi. Delay makes it hard to save. Due to cold: Ding Xiang Shi Di San (Clove and Persimmon Calyx Powder). Due to phlegm: Ju Pi Zhu Ru Tang (Tangerine Peel and Bamboo Shavings Decoction). Postpartum facial/body edema is due to Qi deficiency failing to move water-dampness; differentiate exterior and interior. If caused by early bathing, water entering apertures with heavy body and floating muscle is dampness edema; sweat with Qiang Huo (Notopterygium), Fang Feng, Xiong, Su Ye, Dang Gui, and Fang Ji (Aristolochia). If caused by food/water dampness accumulation with poor urination is water edema; drain with Fu Ling (Poria), Ban Xia (Pinellia), Ze Lan (Lycopus), Che Qian Zi (Plantago Seed), and Mu Tong (Akebia). If limbs swell with stabbing flank/leg pain, retained blood flows into collaterals; use Xiao Tiao Jing San or Niu Xi, Shan Jia (Pangolin Scale), Dang Gui Wei (Angelica Tail), Hu Po (Amber), Hong Hua (Safflower), and Su Mu (Sappan Wood) to dissolve stasis. If Qi fails to transform water causing swelling and dysuria, it is Kidney Yang deficiency; use Shen Qi Tang minus Dan Pi and Shan Zhu Yu to transform Qi and drain water. Postpartum mania due to fright is blood deficiency with Shen not residing; treat primarily with Jia Wei Ba Zhen Tang (Modified Eight Treasure Decoction). Retained blood harassing the Heart/Shield affecting speech and seeing ghosts indicates Heart Protector pathogen; calm and regulate with Fu Ling San, Hu Po San plus Shi Pu Zhu (Acorus Juice). Palpitations and trance indicate unsettled Shen; tonify with Gui Pi Tang (Restore the Spleen Decoction). Postpartum mutism may be due to retained blood closing the Heart Protector; regulate with Qing Hun San (Clear Soul Powder) plus Niu Huang (Calculus Bovis), Dan Shen (Salvia), and Su Mu. Due to phlegm turbidity harassing Heart orifices, use Wen Dan Tang (Warm Gallbladder Decoction) plus Shi Pu Zhu to resolve phlegm. Due to Heart/Kidney Qi deficiency failing to connect to the tongue, use Qi Zhen San (Seven Treasures Powder) to tonify and open. Postpartum excessive sweating leading to spasm, locked jaw, trismus, and limb convulsions is blood deficiency with fierce wind; urgently tonify with Shi Quan Da Bu Tang plus Zhi Fu Zi (Prepared Aconite). Attacking wind leads to death, but ancient methods using Xiao Xu Ming Tang (Minor Extinct Brightness Decoction), Da Dou Zi Tang (Soybean Black Decoction), and Du Huo Tang (Angelica Pubescens Decoction) are very effective. Zhu Fengyi stated: No sweat with aversion to cold is Gang Jing (Yang Spasm), treated with Xiao Xu Ming Tang. Sweat without aversion to cold is Rou Jing (Yin Spasm), Xiao Xu Ming Tang minus Ma Huang (Ephedra) plus Ge Gen (Kudzu Root). Postpartum sweating with dizziness, impending spasm/syncope, is Liver Yin deficiency with moving Wind-Yang; use E Jiao, Sheng Di, Fu Ling, Xiao Mai, Mu Li Fen, and Suan Zao Ren to generate fluids. Postpartum pseudo-stroke with facial deviation and opisthotonos is blood deficiency combined with wind-fire-phlegm; use Xiong Gui Tang (Ligusticum and Angelica Decoction) plus Jing Jie Sui (Schizonepeta Head) fried black soybeans steeped in wine to move blood and dispel wind, or Chuan Xiong San to clear phlegm-fire. Sinews clamped by cold are stiff; clamped by dampness are lax. Blood deficiency with wind-fire entering collaterals mimics stroke; treating postpartum cases as true stroke with Xiao Xu Ming Tang is erroneous. Pseudo-stroke wasting inability to rise is Qi/Blood deficiency with lax sinews; use Zi Yin Da Bu Wan (Nourish Yin Great Tonifying Pill) to strengthen Liver/Kidney. Chi Zong (clonic-clonic movements) involves stiff sinews (Chi) and lax sinews (Zong).
