When the disease is in the Liver, it heals in Summer. If it does not heal in Summer, it worsens in Autumn. If it does not die in Autumn, it stabilizes in Winter, and begins to recover in Spring. When the disease is in the Liver, it heals on Bing-Ding days (Fire days). If it does not heal on Bing-Ding days, it worsens on Geng-Xin days (Metal days). If it does not worsen on Geng-Xin days (In *Suwen*, "does not die"), it stabilizes on Ren-Gui days (Water days), and begins to recover on Jia-Yi days (Wood days). One must avoid exposure to wind. When the disease is in the Liver, the patient feels clear-headed at dawn, symptoms worsen in the late afternoon, and are calm at midnight.
When the disease is in the Heart, it heals in Late Summer. If it does not heal in Late Summer, it worsens in Winter. If it does not die in Winter, it stabilizes in Spring, and begins to recover in Summer. When the disease is in the Heart, it heals on Wu-Ji days (Earth days). If it does not heal on Wu-Ji days, it worsens on Ren-Gui days (Water days). If it does not worsen on Ren-Gui days, it stabilizes on Jia-Yi days (Wood days), and begins to recover on Bing-Ding days (Fire days). One must avoid wearing warm clothes and eating hot food. When the disease is in the Heart, the patient feels clear-headed at noon, symptoms worsen at midnight, and are calm at dawn.
When the disease is in the Spleen, it heals in Autumn. If it does not heal in Autumn, it worsens in Spring. If it does not die in Spring, it stabilizes in Summer, and begins to recover in Late Summer. When the disease is in the Spleen, it heals on Geng-Xin days (Metal days). If it does not heal on Geng-Xin days, it worsens on Jia-Yi days (Wood days). If it does not worsen on Jia-Yi days, it stabilizes on Bing-Ding days (Fire days), and begins to recover on Wu-Ji days (Earth days). One must avoid wearing warm clothes and lying on damp ground (*Suwen* states: avoid warm clothes, full meals, and wet clothes). When the disease is in the Spleen, the patient feels clear-headed during the day, symptoms worsen at dawn (*Suwen* says at sunrise), and are calm in the late afternoon.
When the disease is in the Lung, it heals in Winter. If it does not heal in Winter, it worsens in Summer. If it does not die in Summer, it stabilizes in Late Summer, and begins to recover in Autumn. When the disease is in the Lung, it heals on Ren-Gui days (Water days). If it does not heal on Ren-Gui days, it worsens on Bing-Ding days (Fire days). If it does not worsen on Bing-Ding days, it stabilizes on Wu-Ji days (Earth days), and begins to recover on Geng-Xin days (Metal days). One must avoid cold clothes, frozen drinks, and cold food. When the disease is in the Lung, the patient feels clear-headed in the late afternoon, symptoms worsen at noon, and are calm at midnight.
When the disease is in the Kidney, it heals in Spring. If it does not heal in Spring, it even worsens in Late Summer. If it does not die in Late Summer, it stabilizes in Autumn, and begins to recover in Winter. When the disease is in the Kidney, it heals on Jia-Yi days (Wood days). If it does not heal on Jia-Yi days, it worsens on Wu-Ji days (Earth days). If it does not die on Wu-Ji days, it stabilizes on Geng-Xin days (Metal days), and begins to recover on Ren-Gui days (Water days). One must avoid sexual intercourse, eating hot food, and wearing warm clothes (*Suwen* states: avoid sexual intercourse, hot food, and warm moxa-applied clothes). When the disease is in the Kidney, the patient feels clear-headed at midnight, symptoms worsen during the four seasonal transitions (Chen, Xu, Chou, Wei hours), and are calm in the late afternoon.
When pathogenic factors invade the body, they act according to the cycle of overcoming (Wu Xing). Healing occurs when reaching the organ it generates; worsening occurs when reaching the organ that overcomes it; stabilization occurs when reaching the organ it generates; recovery begins when returning to its own commanding position.
If Cold from the Kidney transmits to the Spleen, it causes abscesses and shortness of breath. If Cold from the Spleen transmits to the Liver, it causes abscesses and tendon spasms. If Cold from the Liver transmits to the Heart, it causes mania and chest diaphragm distension. If Cold from the Heart transmits to the Lung, it forms Fei Xiao (Lung Consumption). Fei Xiao is characterized by drinking one part and urinating two parts; it is a fatal condition and untreatable. If Cold from the Lung transmits to the Kidney, it forms Yong Shui (Surging Water). Yong Shui is characterized by a soft abdomen upon palpation, with water qi lodging in the Large Intestine, causing gurgling bowel sounds when walking quickly, like a bag containing paste. Treatment should primarily target the Lung (*Suwen* refers to this as Water Disease).
If Heat from the Spleen transmits to the Liver, it causes fright and epistaxis. If Heat from the Liver transmits to the Heart, it leads to death. If Heat from the Heart transmits to the Lung, it transforms into Ge Xiao (Diaphragmatic Consumption). If Heat from the Lung transmits to the Kidney, it transforms into Rou Jing (Soft Spasm). If Heat from the Kidney transmits to the Spleen, it transforms into deficiency and intestinal dysentery, which is a fatal and untreatable condition. If Heat from the Bao Gong (Uterus) transmits to the Bladder, it causes strangury and hematuria. If Heat from the Bladder transmits to the Small Intestine, it causes obstruction of the chest and intestines, leading upward to oral ulcers. If Heat from the Small Intestine transmits to the Large Intestine, it forms Zhen Jia (masses), presenting as sinking sensation. If Heat from the Large Intestine transmits to the Stomach, it presents with excessive hunger but scanty urine, named Shi Dan (Food Jaundice), accompanied by dizziness and blurred vision, caused by Jue Ni (Rebellious Qi).
The healthy organs receive Qi from the organ they generate, transmit it to the organ they overcome, lodge it in the organ they generate, and die when reaching the organ that overcomes them. Before death, the disease必先 transmits along its course until it reaches the overcoming organ, then death occurs. This describes the reverse flow of Qi, hence it leads to death.
The Liver receives Qi from the Heart, transmits it to the Spleen, lodges it in the Kidney, and dies when reaching the Lung. The Heart receives Qi from the Spleen, transmits it to the Lung, lodges it in the Liver, and dies when reaching the Kidney. The Spleen receives Qi from the Lung, transmits it to the Kidney, lodges it in the Heart, and dies when reaching the Liver. The Lung receives Qi from the Kidney, transmits it to the Liver, lodges it in the Spleen, and dies when reaching the Heart. The Kidney receives Qi from the Liver, transmits it to the Heart, lodges it in the Lung, and dies when reaching the Spleen. All these are reverse transmissions leading to death. The transmission pattern within one day and night among the five Zang organs is used to predict the timing of death.
Yellow Emperor asked: I received the Nine Needles from you and privately studied various prescriptions. There are methods such as Dao Yin (guiding and pulling), Qi regulation, massage, moxibustion and warming, acupuncture, and herbal medicine. Can one adhere strictly to a single method, or should all be practiced? Qibo replied: These methods are common prescriptions for the masses, not suitable for one person to practice entirely. Yellow Emperor said: This is what is meant by "guarding the One without losing it, and all things are complete." I have heard of the essentials of Yin and Yang, the principles of deficiency and excess, the errors of Qi displacement, and categories of treatable diseases. I wish to hear about the changes of diseases, the spread of pathogenic factors, exhaustion, failure, and untreatable conditions. Qibo said: Excellent question! The Dao is clear as waking in the morning, and obscure as sleeping at night. If one can accept and apply it, Shen (Spirit) will form together with it. If one fully applies it, Shen will naturally obtain it. The principle of generating Shen can be written on bamboo and silk, but cannot be passed down to descendants. Yellow Emperor asked: What is meant by waking in the morning? Qibo said: Understanding Yin and Yang is like resolving confusion, like waking from drunkenness. Yellow Emperor asked: What is meant by sleeping at night? Qibo said: Silent and formless. Damaging hair and pores, Zheng Qi (Upright Qi) collapses. Pathogenic factors spread diffusely, blood vessels stagnate. Da Qi (Great Qi) enters the Zang organs, causing abdominal pain and lower discharge. This can lead to death, not to life. Yellow Emperor asked: How does Da Qi enter the Zang organs? Qibo said: If the disease starts in the Heart, there is heart pain. After one day, it reaches the Lung causing cough. After three days, it reaches the Liver causing rib-side fullness. After five days, it reaches the Spleen causing blockage, heaviness of the body. If not resolved in three days, death occurs. Death happens at midnight in winter and noon in summer. If the disease starts in the Lung, there is wheezing and cough. After three days, it reaches the Liver causing rib-side fullness. After one day, it reaches the Spleen causing body pain. After five days, it reaches the Stomach causing distension. If not resolved in ten days, death occurs. Death happens at sunset in winter and sunrise in summer. If the disease starts in the Liver, there is headache, vertigo, and rib-side fullness. After one day, it reaches the Spleen causing body pain. Diseases transmitted in sequence like this all have a fixed time of death and cannot be treated with acupuncture.
