(With supplementary additions)
Fetal Cold Syndrome. The fetus is cold in the womb, and after birth, it is exposed to cold again. Within half a day to one day, the entire body turns blue-green like indigo dye. The mouth does not suckle milk. The four limbs must be cold.
Initially there are crying sounds, but later the infant stops crying and falls into a coma; this is Fetal Cold Syndrome. Observe that the infant's eyes and nasal tip show no yellow color, and there is no blowing or sighing sound from the mouth. It is definitely Fetal Cold Syndrome. First, firmly grasp and manipulate the Jiangdao (Guide) and Weiling (Spirit of Mightiness) acupoints bilaterally, and also firmly grasp the Kunlun (BL-60) acupoint. If the infant makes a sound, immediately apply moxibustion with lantern fire fifteen times at each point to terminate the condition (method seen previously). Or if no sound comes out, use these same points, then the sound will surely emerge. Milk sucking will resume, and the cyanotic color will gradually recede. However, this syndrome requires prevention of vomiting. In Fetal Cold Syndrome, all internal organs are cold; stomach cold prevents food acceptance, hence vomiting inevitably occurs. Use Ren Shen (Ginseng Radix et Rhizoma, 1 fen), Jie Geng (Platycodonis Radix, 1 qian), Bai Zhu (Atractylodis Macrocephalae Rhizoma, 5 fen), and Huo Xiang (Pogostemonis Herba, 5 fen), decocted in water and taken orally. It will heal itself. There are also cases occurring between two or three days after birth, where there is cold in the fetus but no external wind-cold invasion.
Fetal Heat Syndrome. The facial complexion is deep red and dry, feeling as if hot breath fills the mouth. Or the tongue is swollen and red-purple, and the inner eyes are red. Or there is constipation and scanty dark urine.
These are all signs of Fetal Heat Syndrome. If the gums are swollen and hard, preventing suckling, pierce the area between the upper and lower teeth at the bone junction with a silver hairpin until bleeding occurs, then suckling will naturally resume. Subsequently, use Lian Qiao (Forsythiae Fructum, 1 qian), Mu Tong (Akebiae Caulis, 1 qian), and Gan Cao (Licorice Root, 5 fen), decocted in water and taken orally.
Fetal Convulsion. "Jing" (Startle) and "Feng" (Wind) have different names but identical symptoms. When a newborn has a cyanotic face, locked jaw, convulsions of hands and feet, fever, and stiff back, this is Fetal Convulsion Syndrome.
If the eyes stare straight or dart around, it is most difficult to treat. One should first grasp the Jingmen (Gate of Startle) and Weiling acupoints. Also grasp the Kunlun (BL-60) acupoint. After a moment, bend the infant's little finger and heavily rub the Wailaogong (Extra Point EX-UE8) acupoint. Then use lantern fire moxibustion to calm the startle.
Umbilical Wind (Neonatal Tetanus). When an infant is born and the umbilical cord is cut, wind pathogen enters the abdomen through the umbilicus. Wind nature is rapid; symptoms appear within three days. After seven days, it is no longer Umbilical Wind.
At the onset of Umbilical Wind, suckling is looser than before. A sudden yellow color appears near the inner canthi close to the glabella. Immediate treatment is required; it is easiest to treat at this stage. If the yellow color spreads to the nose, treatment is still easy. If it reaches the Renzhong (GV-26) and Chengjiang (CV-24) acupoints, treatment becomes slightly difficult. If the mouth is not puckered but there is slight blowing/sighing, it is still treatable. When the lips and mouth tighten and lock, and the tongue becomes stiff, it is untreatable. Upon seeing yellow color at the inner canthi and Renzhong, but without lip puckering, bend the infant's little finger and rub the Wailaogong (EX-UE8) acupoint. Immediately apply lantern fire moxibustion: one burn at the Fontanelle (Baihui GV-20 area), Meixin (Yintang EX-HN3), Renzhong (GV-26), Chengjiang (CV-24), both Shaoshang (LU-11) acupoints. Six burns on the umbilical ring. For uncut cord, ignite fire at the cord base for one burn. For cut cord, one burn at the stump. Total thirteen burns. Wind will stop and yellow color will recede.
Acute Convulsion. Pediatric acute convulsion is caused by hearing loud noises or being startled, leading to spasms. Spasms stop and the child returns to normal. This is because heat arises in the Heart. Symptoms include fever, red face, desire for drinks, hot breath, and yellow-dark urine and stool. Severe cases involve convulsions due to extreme heat generating wind, characterized by excess Yang and deficiency of Yin. Use Li Jing Wan (Relieve Startle Pill).
Fever without convulsions, coma, and lack of awareness indicates Heart Heat. Treatment should use Ban Xia (Pinelliae Tuber), Chen Pi (Citri Reticulatae Pericarpium), Jie Geng (Platycodonis Radix), Gan Cao (Licorice Root), Lian Qiao (Forsythiae Fructum), and Mu Tong (Akebiae Caulis). Equal parts, decocted and taken orally.
Li Jing Wan (Relieve Startle Pill)
Tian Zhu Huang (Bambusae Concretio Silicea, 2 qian), Qing Fen (Calomel, 1 qian), Qing Dai (Indigo Naturalis, 1 qian), Hei Qian Niu (Semen Pharbitidis, stir-fried, 5 qian). Grind into powder. Form pills the size of soybeans with honey. Take one pill per year of age. Wash down with Bo He Tang (Mint Decoction).
Chronic Convulsion (also known as Chronic Spleen Wind). Pediatric chronic convulsion may result from damage to the Spleen and Stomach by medication after illness, or from prolonged nursing causing Yin depletion, prolonged vomiting damaging the Stomach, or prolonged diarrhea severing the Spleen. Hence it is called Chronic Spleen Wind Syndrome. Symptoms include drowsiness with exposed eyes, spiritless eyes with excessive tearing, gurgling sound in the throat, cold extremities, and twitching. Stool is greenish, while urine is clear and profuse. This is Spleen Deficiency. Some infants turn their eyes back and clench fists, resembling convulsions, hence named convulsion. Even with pushing/manipulation or moxibustion, there is no startle to treat, no wind to dispel, no phlegm to remove, and no heat to resolve. It is actually only dry phlegm in the Spleen and fluctuating deficient heat. Treatment should use Liu Jun Zi Tang (Six Gentlemen Decoction) and similar formulas.
Convulsions. In convulsions from startle or epilepsy: Boys look left silently and right with sound, due to mutual overcoming.
For boys, curling the thumb into the index finger is favorable (Shun); extending outward is unfavorable (Ni). For girls, curling into the index finger is unfavorable; extending outward is favorable.
Convulsion from Wind Invasion: Hot breath, yawning, restlessness, trembling of hands and feet.
Convulsion from Food Retention: Warm body, excessive sleep, or vomiting with loss of appetite.
Convulsion within 100 days: True cases die within two or three episodes; false cases recur frequently but do not die. True cases arise from internal startle/epilepsy; false cases from external wind-cold invasion. Qi and Blood are not yet solid, unable to withstand, hence convulsions occur.
Prognosis of Startle/Epilepsy. If phlegm obstruction causes unconsciousness and no hand convulsions, firmly grasp the Jingmen and Weiling acupoints bilaterally. If there is no teeth grinding, head shaking, staring straight, or struggling vocalization, place the infant facing you. Straddle the infant's back with your hands, thumbs forward, and press-hardly rub the Fei Shu (BL-13) acupoints with your second fingers. Although no sound comes out, there is a struggling motion, seemingly having sound but no voice. This indicates Lung obstruction by phlegm, like a bell filled with solid matter that cannot resonate even when struck. This is definitely a livable case. Moxibustion Fei Shu (BL-13) with three moxa cones urgently. If startle is relieved and consciousness returns, the patient is saved. If the infant is thin, phlegm is not severe, unconscious, opens eyes looking at people, and upon grasping Jingmen and Weiling bilaterally feels no pain, shows no struggling sound, only teeth grinding and head shaking, this indicates Lung Meridian exhaustion; treatment is futile.
Startle/Epilepsy begins with fearful crying.
Later, moxibustion at the center of the whorl hair on the vertex (also known as Niwan Acupoint) and the blue vein behind the ear (three cones each).
Wind Epilepsy begins with flexing the index finger as if counting.
Needle Sizhukong (TE-23), Shenting (GV-24), Baihui (GV-20). Moxibustion at Shenque (CV-8).
Luo Li (Scrofula). Wind-heat toxic pathogens interact with Qi and Blood, stagnating and forming nodules like beads strung together between the ears and neck. Swollen, hard, white, and rooted nodules are Luo Li.
If ulcerated into malignant sores, use lantern fire moxibustion. If Luo Li is on the left, moxa the left side; if on the right, moxa the right side. Start anteriorly from above the neck below the earlobe, leaving a six-fen gap, applying point-by-point downwards past the nipple line through the axilla, circling to Fei Shu (BL-13) up to behind the ear. Moxa around the Luo Li once more, filling gaps along the original path to cure. If the nodule is movable, it is not Luo Li. Initial redness and swelling indicate Carbuncle/Furuncle, not to be treated as Luo Li.
Huo Luan (Colic with Vomiting and Diarrhea)
Kunlun (BL-60), Shui Fen (CV-9), Tian Shu (ST-25).
Vomiting Milk
Moxibustion at Zhong Ting (CV-17) with one cone.
Passing Feces from Mouth
Caused by the mother consuming cold-cooling foods.
Moxibustion at Zhong Wan (CV-12) with seven cones.
Fullness in Hypochondrium, Dysentery, Heavy Body Unmovable. Pi Accumulation, Abdominal Pain, Loss of Appetite. Malaria with chills/fever or abdominal distension radiating to back, increased intake, gradual emaciation. Moxibustion 7 cones at 1.5 cun lateral to the spinous process of the 11th thoracic vertebra. Jaundice: 3 cones (likely Pi Shu BL-20 bilateral).
Abdominal Distension with Visible Veins, Hard as Iron. Moxibustion 2 cones at 5 fen distance above, below, left, and right of the umbilicus. Condition resolves.
Gan Eye (Nutritional Keratomalacia). Caused by irregular eating habits leading to food retention damaging the Spleen, large abdomen, yellow face. Fever in the afternoon, prolonged loose stools. Severe diarrhea leads to thirst. Red sclera, gradually covered by pterygium obscuring the cornea. Protrusion like black beans or shiitake mushrooms indicates Gan Eye. (Supplementary addition)
Moxibustion at He Gu (LI-4) bilateral, one cone each.
Chicken Chest (also known as Turtle Chest). Caused by coughing and wheezing leading to Lung Qi fullness attacking the chest and diaphragm, gradually forming this syndrome.
Zhong Fu (LU-1), Dan Zhong (CV-17), Ling Dao (HT-4), Zu San Li (ST-36).
Turtle Back (also known as Tortoise Back). Caused by exogenous wind invading the back, transmitting to the marrow. Thus the back protrudes like a turtle. Or prolonged coughing leads to Lung deficiency failing to nourish Kidney. Kidney governs bones; wind-cold enters bones via deficiency. Marrow essence and blood cannot circulate, causing spinal curvature and hunchback.
Jian Zhong Shu (SI-11) (for treating cough), Gao Huang Shu (BL-43), Shen Shu (BL-23).
Pi Qi (Accumulation) persisting long without resolution.
Zhang Men (LR-13) three cones. Ming Men (GV-4) posterior to umbilicus on spine, three cones (Highly effective for Malaria Mass).
Cold Dysentery in late autumn.
Three cun below the umbilicus (Qihai CV-6), seven cones or age-dependent cones.
Rectal Prolapse with Bleeding. Ineffective in late autumn.
Place ginger slice on umbilicus, moxa three cones. Qian Jin Fang says: Moxa age-dependent cones.
Gui Wei (Du-1, also known as Chang Qiang), three cones.
Rectal Prolapse is due to sinking of Lung Qi. Concurrently use Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) for several doses for efficacy. For chronic non-healing cases, moxa Baihui (GV-20).
Yin Swelling.
Moxibustion at Kunlun (BL-60) with three cones.
