Discussion: The intestines and stomach are the granaries, opening and closing at proper times. Dysentery occurs because the intestines and stomach lose their guard, the gates are not secure, due to mixing of water and grain, improper transformation and transportation, and failure of qi to consolidate. Alternatively, spring wind injury lingers, damaging the spleen. Spleen belongs to Earth and dislikes Wood. Connected by membranes, the spleen and stomach become weak, unable to transform water and grain, which pour into the intestines. The deficient intestines cannot contain them, causing diarrhea. If the body is thin with a large abdomen, distended, and food passes immediately after eating, this Bai Zhu Wan with Fu Zi Tang is suitable.

[Jiang Fu Tang]
If sudden profuse watery diarrhea, green or white, with cramping abdominal pain, cold extremities, fine pulse nearly absent, and gradual shortness of breath, this is caused by cold qi suddenly injuring yang, being repelled by yin, failing to consolidate qi. If yang recovers, life is preserved; otherwise, death ensues. Jiang Fu Tang and Jin Ye Dan (later formula) are suitable. If still uncontrolled, moxa Qihai (CV6) 100 cones.

[San Wei Huang Lian Wan]
If dysentery is red-yellow, occurring dozens of times day and night, without cold extremities, with chest stuffiness, occasional slight chills, and worsening after taking warm medicines with a deep, solid, large pulse, this is pathogenic factor stagnation in the intestines and stomach, compounded by undigested food accumulation. Water and grain pour downward. First use this San Wei Huang Lian Wan. If ineffective, use the following formula.
Huang Lian (Coptidis Rhizoma), Wu Zhu Yu (Evodiae Fructus), Bai Shao Yao (Paeoniae Radix Alba).
Divide equally, grind into fine powder, mix with vinegar-boiled wheat paste to make pills the size of wu zi. Take 30 pills each time with rice drink.
[Zhu Fen Dan]
If dysentery is pure pus, red or white, with cramping abdominal pain that stops after defecation, and a tight, large, rapid pulse, this is due to improper diet remaining inside, qi conflict producing white discharge, heat conflict producing red-water discharge. Grain and intestinal fluids rot into pus. New grain transmission is blocked, new and old conflict causes stagnation pain. When qi moves and pain subsides slightly, it is called "accumulation dysentery". Take this.
Ba Dou (Crotonis Fructus) (1 qian), Fen Shuang (Arsenic Trioxide processed), Sha (likely Realgar or Cinnabar processed) (each 0.5 qian).
Grind evenly, melt 2 qian of yellow beeswax, add drugs, stir well. Make pills the size of mung beans. Take one pill with rice drink. If unknown effect, administer again.
[Xiang Ru San]
Treats irregular diet, excessive raw/cold/fishy food, irregular lifestyle, sleeping in damp places, or cooling off in wind. Wind-cold qi returns to the spleen and stomach, which cannot digest food, causing true and evil conflict, intestinal weakness, and dietary chaos leading to vomiting, diarrhea, pain, cholera, qi rebellion. Cases include heart pain with vomiting first, abdominal pain with diarrhea first, simultaneous vomiting and diarrhea, fever/headache/body ache with vomiting/diarrery, irritability, stabbing heart/abdominal pain, muscle cramps, vomiting without substance, cold extremities with near-absent pulse, or irritability/coma nearing death. This formula addresses all.
